4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The current condition of these two small streams and adjacent riparian area is very poor. Bank erosion is evident throughout the proposed project reach and active erosion continues to
erode multiple mass wasting banks at the site. The health of the riparian corridor is poor; little riparian buffer or bedform diversity is currently present. The land use surrounding the
proposed project consists mostly of pasture, hayland, and forests.

The landowner has been approved for an Environmental Quality Incentives Program (EQIP) contract with the US Department of Agriculture's Natural Resources Conservation Service
(NRCS) to stabilize and restore this stream and to plant native riparian vegetation. Currently, the streambanks are eroding causing impairment to water quality and instream aquatic
habitat. The proposed stream restoration project will restore about 672 feet of this stream by improving channel and bank stability, as well as installation of trees and shrubs to create a
riparian buffer consisting of native species.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
The proposed stream restoration project will restore approximately 672 feet of two small tributaries with the goal of improving natural ecological conditions and streambank stability. The
stream will be restored by grading the channel and streambanks to a naturally stable morphology and adding native vegetation adapted to site conditions for bank protection and soil
strength. Natural channel design in -stream structures will be used to maintain grade control, enhance bedform diversity, and protect streambanks from erosive hydraulic forces. Erosion
control matting, live stakes, bare root seedlings, vegetation plugs, and temporary and permanent seeding will be established to support long term stream health. The project will have a
positive impact on the downstream watershed by reducing sediment loading and creating a shaded woody stream buffer to help maintain water temperature. Equipment used to construct
the project will include a tracked hydraulic excavator with a hydraulic thumb, loaders, and dump trucks/trailers.

Impacts to the 672 feet of stream channel include the installation of j-hooks, log steps, and constructed riffles. These structures are needed to maintain
grade, reduce shear stress along the streambanks, and to enhance bedform diversity. This will result in a long-term positive impact to instream aquatic
habitat.

Impacts will be minimized by maintaining the existing channel and minimal existing buffer as much as possible.

Construction impacts will be minimized by limiting in -channel work to low -flow conditions and working in the dry as much as possible. Streambanks and
other disturbed areas will be stabilized as soon as possible using seed, straw, and biodegradable erosion control matting. Appropriately sized equipment
will be used to minimize soil compaction. Experienced stream restoration contractors will work efficiently to minimize the time of disturbance.

SUMMARY OF PROPOSED PROJECT:
This stream restoration project will address instabilities along approximately 672 feet of
two small tributaries to the Middle Prong Roaring River. The channel is migrating
laterally in multiple meander bends. This process is actively eroding vertical mass
wasting banks throughout the proposed project reach. Instabilities exist throughout the
reach. Additionally, the stream does not hove adequate access to a floodplain.
The objective of the proposed project will be to use natural channel design
methodology to address the stream's dimension, pattern, and profile to improve
sediment transport and stop bank erosion. Pattern or alignment will be corrected to a
tolerable range and he/d in p/ace with multiple structures including bou/der steps, /og
steps, and j-hooks. These structures will also maintain a stable channel dimension
and profile created through construction activities. Native herbaceous and woody
vegetation will be installed to create a high quality riparian corridor
GENERAL PROJECT SPECIFICATION AND NOTES
1. EXTREME CARE AND DILIGENCE SHALL BE EXERCISED BY THE CONTRACTOR TO ASSURE THE SAFETY OF PERSONS, ANIMALS, AND PROPERTY. THE
CONTRACTOR SHALL MAINTAIN ALL LIGHTS, GUARDS, SIGNS, TEMPORARY PASSAGES, OR OTHER PRECAUTIONS NECESSARY FOR THE SAFETY OF ALL
PERSONS. THE CONTRACTOR SHALL ABIDE BY ALL SAFETY RULES AND CONSTRUCTION CONDITIONS REQUIRED BY GOVERNMENTAL AUTHORITIES AND
OTHER ENTITIES, INCLUDING RAILROADS, SO THE PUBLIC IS SAFEGUARDED FROM ACCIDENTS AND DELAYS. GUARDS AND FLAGS REQUIRED BY

NAVD88.
7. EXISTING GROUND SURFACES WERE DEVELOPED BASED ON COMBINING AND MERGING LiDAR DATA AND A SITE SURVEY. SOME CHANGES MAY HAVE
OCCURRED SINCE THE SURVEY WAS COMPLETED. VERTICAL ACCURACY OF EXISTING GROUND IS ± 1 FOOT.
8. THE PROPOSED ELEVATIONS AND GRADES SHOWN HEREIN ARE BASED ON THE DATA DESCRIBED ABOVE THAT ENCOMPASSES THE EXISTING GROUND

DOCUMENTS DEPENDING ON SITE CONDITIONS OR PROJECT NEEDS. MINOR VARIATION(S) OR ADAPTATION(S) OF THE PROPOSED WORK SHOWN ON THE
PLANSHEETS AND/OR DETAILS ARE CONSIDERED INCIDENTAL TO THE WORK.
11. WHERE PRACTICABLE, EXISTING TREES AND VEGETATION SHOULD BE LEFT IN PLACE TO FACILITATE NATURAL REGENERATION AND SOIL STABILIZATION.
12. THE CONTRACTOR SHALL STAKE OUT THE PROPOSED STREAM CENTERLINE IN APPROXIMATELY 500 FT SECTIONS USING SURVEYING EQUIPMENT FOR
REVIEW BY THE ENGINEER BEFORE BEGINNING EXCAVATION AND GRADING. DEPENDING ON ENCOUNTERED CONDITIONS SOME SHIFTING OF THE STREAM
ALIGNMENT MAY BE NECESSARY. PRIOR TO CLEARING AND GRUBBING, THE CONTRACTOR SHALL MARK THE LIMITS OF CLEARING NEAR TREES FOR
VERIFICATION OF INTENT BY THE ENGINEER. SOME MINOR ADJUSTMENT OF CHANNEL ALIGNMENT MAY BE REQUIRED TO PRESERVE TREES OR MINIMIZE
IMPACT TO TREES.
13. ANY HARVESTING OF TREES FROM ONSITE MUST BE APPROVED BY THE ENGINEER. CONTRACTOR SHALL MINIMIZE, TO THE MAXIMUM EXTENT POSSIBLE,
IMPACTS TO THE ADJACENT TREES.
14. CONTRACTOR SHALL USE AN EXCAVATOR WITH A HYDRAULIC THUMB TO INSTALL IN -STREAM STRUCTURES.
15. ELEVATIONS OF TRIBUTARIES AT CONFLUENCES MAY NEED TO BE ADJUSTED TO MEET CONSTRUCTED CONDITIONS. ADJUSTMENTS SHALL BE MADE IN
CONJUNCTION WITH THE ENGINEER. PROFILES MAY NEED TO BE ADJUSTED TO AVOID ABRUPT CHANGES IN ELEVATION. ADJUSTMENTS SHALL BE MADE IN
CONJUNCTION WITH THE ENGINEER.
16. STREAM RESTORATION WORK SHALL BE IMPLEMENTED BY FIRST GRADING THE FLOODPLAIN ADJACENT TO THE CHANNEL TO THE ELEVATIONS AND
GRADES SPECIFIED IN THE PLANSHEETS. THE PROPOSED STREAM CHANNEL SHALL THEN BE EXCAVATED TO THE CHANNEL CROSS-SECTION GEOMETRY
AND LONGITUDINAL PROFILE IN THE CONSTRUCTION DOCUMENTS. THIS CHANNEL WORK SHALL BE DONE WITH LOW GROUND PRESSURE TRACK
EQUIPMENT. PLANSHEETS PROVIDE DIMENSIONS, ELEVATIONS AND SLOPES TO AID IN CONSTRUCTION OF THE CHANNEL. THE THALWEG CAN FIRST BE
EXCAVATED TO THE ELEVATION SPECIFIED IN THE LONGITUDINAL PROFILE AND EXCAVATION AND FINE GRADING OF THE CROSS -SECTIONS SHALL THEN BE
PERFORMED.
17. BANKFULL CHANNEL DIMENSIONS WILL BE HELD TO THE DIMENSIONS SHOWN ON THE TYPICAL CROSS-SECTION PLANSHEETS. ELEVATIONS SHALL BE

GENERAL RE —VEGETATION PLAN NOTES
1. SOIL PREPARATION ELEMENTS, TEMPORARY AND PERMANENT SEED AND GROUND
COVER SHALL BE SPREAD OVER ALL AREAS WITHIN THE LOD THAT ARE DISTURBED
DURING CONSTRUCTION.
2. THE LANDOWNER PREFERS THAT ANY REVEGETATION OUTSIDE OF THE RIPARIAN
PLANTING AREA CONSIST OF TALL FESCUE, LIME, AND FERTILIZER.
3. LIVE STAKES, BARE ROOT STOCK AND CONTAINERIZED PLANTS SHALL BE EVENLY
DISTRIBUTED THROUGHOUT THE RIPARIAN PLANTING AREA AS DIRECTED BY THE
ENGINEER.
4. ADJUSTMENTS TO THE VEGETATION PLAN SHALL BE MADE ONLY IF APPROVED BY THE
ENGINEER.
5. LONG-TERM SUCCESS OF THE PROJECT WILL BE DEPENDENT ON THE LANDOWNER'S
ABILITY TO MONITOR AND REMOVE BEAVERS FROM THE SITE, PROTECTION AND

LIVE STAKES
11. LIVE STAKES MUST BE DORMANT WHEN CUT. KEEP LIVE STAKES MOIST UNTIL PLANTING.
THE STAKE SHOULD BE PREPARED WITH THE BUDS POINTED UP, AND THE BOTTOM
SHOULD BE CUT AT AN ANGLE FOR EASY INSERTION INTO THE GROUND.
12. LIVE STAKES SHOULD BE PLACED WITH 3 TO a OF THE LENGTH OF THE STAKE BELOW
GROUND AND ANGLED DOWNSTREAM. ENSURE THE BASE OF THE LIVE STAKE WILL REACH

14. STOCK SHALL HAVE BEEN GROWN IN A CONTAINER LONG ENOUGH FOR THE ROOT

15. CONTAINER PLANTS WILL NEED TO BE WATERED REGULARLY AND PLACED IN SHADY
