Please find attached a copy of the completed form entitled "Stormwater Compliance
Inspection Report" The report summarizes the findings of a recent inspection of the
project's stormwater controls conducted on April 29, 2010 to determine compliance with
Stormwater Permit Number SW8 910501 MOD issued on December 21, 2010
The required actions that are necessary to bring this site into compliance are noted on
the attached inspection report Please inform this Office in writing before Friday, May
28, 2010, of the specific actions that will be undertaken and the time frame that will be
required to correct the deficiencies Failure to provide the requested information, when
required, may initiate enforcement action including the assessment of civil penalties of
up to $10,000 per day
As indicated in the attached inspection report the following issues must be resolved
Ensure that the BUA from the previously unpermitted parking lot is incorporated into
the current modification request, or if it was included in a previous modification,
provide documentation that this was done
2 Ensure that the drawings are modified to include all inlets to the pond, including
where they come from If either of the outfalls are draining something other than
stormwater they may require coverage under a separate permit
3 When the outlet structure is modified ensure that it is installed properly
4 Ensure that trash does not accumulate in the pond

Inspection Summary
The following deficiencies were noted during the Inspection
1 There Is an approximately 20,800 square foot parking tot located just North of the permitted parking lot In front of the
Odell Williamson Auditorium that does not appear to have been approved In either of the last two Iterations of this permit
2 There are two additional Inlets draining Into the northern leg of the pond from what was shown In the plans approved on
December 22, 2010
3 The outlet protection screen shown on the approved plan details does not appear to have been Installed As a result
the orifice was clogged
4 Refuse Is making it Into the Northern leg of the pond from the adjacent dumpster
5 The pond orifice was clogged, apparently by water hyacinth, and as a result the water level was being held about 6

Comment Refuse is making it into the Northern leg of the pond from the adjacent
dumpster
The pond orifice was clogged, apparently by water hyacinth, and as a result the water
level was being held about 6 inches above the design depth, and was not flowing at all
Woody vegetation has been eliminated from the slopes of the pond, but there is a large
stand of cattails that are covering about 40 percent of the pond's surface When they
reach 50 percent coverage of the pond they will need to be removed from the pond with
the exception of the vegetative shelf and forebay berm

An inspection of the orifice pipe at the Odell Williamson Auditorium (storm -water
retention pond) January 2, 1995 revealed a blockage Flow was restored at 2 00 p m ,
and the pond level was closely monitored for four days The average drop indicated
approximately 3" per day
We receivei2+" of rain January 7-8 which raised the pond level M 6 1/2" The
orifice is now functioning as designed
Based on my calculations throughout the seven day monitoring period, I believe
that the 1" orifice is sufficient to maintain the desired storage capacity and run-off rate

The problem observed by NC DEI-INR may be due to a blockage or clot in the 1"

In follow up to our telephone conversation on December 8, 1994, Talbert & Bright, Inc, would like
to respond to your stormwater compliance inspection report dated November 8, 1994 As we
discussed, we will notify the owner that an inspection is needed on the 1" orifice in order to
determine whether or not there is a clog or other type of blockage that would prevent the pond from
drawing down to the proper level after five days
We make these recommendations based on the original design calculations The calculations were
double checked and show that a 1 " orifice should drain a volume of 10,954 cubic feet in the required
two day minimum However, since it was observed that the drawdown level was not adequate after
five days, we believe that the 1" orifice is blocked
If, after inspection, it is determined that the onfice is not blocked, we will discuss with you alternate
options Until such time however, we will notify you of the result of the inspection

1 A review of the files shows that tlus Office was concerned about the small onfice size due
to the amount of volume being provided in the pond (twice as much as required) Staff
suggested at that time to increase the 1" orifice to between 1 2" and 1 9" in order to draw
down the proposed volume The engineer responded that he believed that the required
volume of 10,954 cubic feet would exit the pond in less than the required 2 day mimmum
if a larger onfice were put in to draw down the 21,200 cubic feet of storage provided
Apparently, this argument was accepted and the pond was certified Now, however, the
accepted thinking is that the volume that is provided will be used to determine the correct
onfice size so that the volume being held can exit the pond before the next storm event
If water is allowed to remain in the pond beyond the 5 day limit, then the 10,954 cubic
feet necessary to store the 1" runoff volume may not be available, and untreated runoff
may exit the pond Please replace the existing 1" onfice with a larger one between 12
and 1 9", preferably closer to the larger end (re , 1 75")

We are making progress in our efforts to repair the erosion control
measures of the above referenced project
The site subcontractor, who worked under the previous General
Contractor, has agreed to return to the site and repair the damaged areas
We have met with him and with a representative of the Division of State
Construction at the jobsite, to discuss the scope of work Our
specifications call for the installation of a temporary, erosion -control
seeding However, we are negotiating with the Contractor for the
Owner to provide a mix of permanent seed to be sewn, rather than a
temporary grass
Whether they use a temporary -type of grass, or a permanent mix
provided by the Owner, this work will be completed soon
We appreciate your assistance in this matter, and trust that the work,
