Please see the attached operating agreement for Bell Carolina LLC which lists Ron Igarashi as on of the officers
of the LLC Ron is listed as the Secretary and signs all renewals for us
The Member of the LLC is Bell American Group LLC it is an entity not an individual which Ronald Igarashi is
part of as well Ron's signature should suffice
Renewal are processed for our company out of an Oregon office the Post office box is that
address Additionally we have listed the Ohio office as a street address Both of those addresses are for our
company
The street address DOES Match what the secretary of state shows

The Wilmington Regional Office received and accepted a State Stormwater Management Permit Renewal
Application for the subject project on August 2, 2021 A preliminary in-depth review of that information has
determined that the application is not complete and that additional information is needed To continue the review,
please provide the following additional information in a single, hard copy submittal package
15A NCAC 02H 1040(1)(a) and 1042(2)(b)(u) The application was signed by Ronald Igarashi, however,
the provided documentation from the NC Secretary of State does not identify Mr Igarashi as a manager of
this LLC In the case of a limited liability corporation (LLC), the application must be signed by a manager or
company official as defined in GS 57D Please either
a Have one of the identified members of the LLC sign the application documents, or
b The signature by other agents shall be accepted on the application only if accompanied by a letter

Central Office (Urbanizing Areas Ph 2, USMP, Non -Coastal HQW/ORW) Only applications packages that
