State Stormwater Management Permit Application for Broad & Gales Creek Fire Department Addition on June 22, 2012 Staff review of the plans and specifications has determined that the protect, as proposed, will comply with the Stormwater Regulations set forth in Session Law 2008-211 and Title 15A NCAC 2H 1000 We are forwarding Permit No SW8 120218 dated June 26, 2012, for the construction, operation and maintenance of the BMP's and built -upon area associated with the subject project This permit shall be effective from the date of issuance until June 26, 2020, and shall be subject to the conditions and limitations as specified therein Please pay special attention to the conditions listed in this permit regarding the Operation and Maintenance of the BMP(s), recordation of deed STORMWATER MANAGEMENT PERMIT HIGH DENSITY COMMERCIAL DEVELOPMENT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Broad & Gales Creek Community Association Broad & Gales Creek Fire Department Addition 3010 Hwy 24, Newport, Carteret County FOR THE construction, operation and maintenance of two (2) infiltration basins in compliance with the provisions of Session Law 2008-211 and 15A NCAC 2H 1000 (hereafter collectively and separately referred to as the "stormwater rules') and the approved stormwater management plans and specifications and other supporting data as attached and on file with and approved by the Division of Water Quality and STANDARDS 1 This permit is effective only with respect to the nature and volume of stormwater described in the application and other supporting data 2 This stormwater system has been approved for the management of stormwater runoff as described in Section 17 on page 3 of this permit The infiltration basins labeled as Basin 1 and Basin 2 have been designed to treat the runoff from 35,790 square feet and 4,017 square feet of built -upon area, respectively 3 The tract will be limited to the amount of built -upon area indicated in Section 18 on page 3 of this permit, and as shown on the approved plans 4 All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements The final plats for the project will be recorded showing all such required easements, in accordance with the approved plans 5 The runoff from all built -upon area within the permitted drainage area of this project must be directed into the permitted stormwater 16 3 9 of the BMP manual, to infiltrate a 10-year, 24-hour storm without discharging the system using an infiltration rate of one-half of the infiltration rate reported in the sods report Therefore, no bypass, no vegetated filter strip, and no additional storage volume are required II SCHEDULE OF COMPLIANCE 1 The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface 2 During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately 3 The facilities shall be constructed as shown on the approved plans This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data 4 If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility 5 A copy of the approved plans and specifications shall be maintained on file by the Permittee at all times Page 3 of 7 State Stormwater Management Systems Permit No SW8 120218 6 The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to a Semiannual scheduled inspections (every 6 months) b Sediment removal c Mowm.9 and revegetation of slopes and the vegetated filter d Immediate repair of eroded areas e Maintenance of all slopes in accordance with approved plans and specifications f Debris removal and unclogging of bypass structure, infiltration media, flow spreader, catch basins, piping and vegetated filter g A clear access path to the bypass structure must be available at all times 7 Records of maintenance activities must be kept for each permitted BMP The reports will indicate the date, activity, name of person performing the work and what actions were taken Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation Any deviations from the approved plans and specifications must be noted on the Certification A modification may be required for those deviations Access to the stormwater facilities shall be maintained via appropriate easements at all times 10 The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below a Any revision to any item shown on the approved plans, including the stormwater management measures, built -upon area, details, etc b Project name change c Transfer of ownership d Redesign or addition to the approved amount of built -upon area or to the drainage area e Further development, subdivision, acquisition, lease or sale of any, all or part of the project area The protect area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought f Filling in, altering, or piping of any vegetative conveyance shown on the approved plan 11 The permittee shall submit final site layout and grading plans for any permitted future areas shown on the approved plans, prior to construction 12 The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made Page 4 of 7 State Stormwater Management Systems Permit No SW8 120218 III GENERAL CONDITIONS Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit This permit is not transferable to any person or entity except after notice to and approval by the Director At least 30 days prior to a change of ownership, or a name change of the project or of the permittee or a mailing address change, the permittee shall submit a completed and signed Name/Ownership Change form to the Division of Water Quality, accompanied by the appropriate documentation as listed on the form The approval of this request will be considered on its merits and may or may not be approved The permittee is responsible for compliance with all permit conditions until such time as the Division approves a transfer request 4 Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215 6A to 143-215 6C The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal) having jurisdiction In the event that the facilities fad to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems The permittee grants DENR Staff permission to enter the property during normal business hours for the purpose of inspecting all components of the permitted stormwater management facility 8 The permit issued shall continue in force and effect until revoked or terminated The permit may be modified, revoked and reissued or terminated for cause The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition 9 Unless specified elsewhere, permanent seeding requirements for the stormwater controls must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual 10 The issuance of this permit does not prohibit the Director from reopening and modifying the permit, revoking and reissuing the permit, or terminating the permit as allowed by the laws, rules and regulations contained in Session Law 2008- 211, Title 15A NCAC 2H 1000, and NCGS 143-215 1 et al 11 The permittee shall submit a permit renewal application at least 180 Groundwater was encountered below about 92 inches beneath the surface The seasonal high water table at this site is about 78 inches beneath the surface where soil moisture significantly increased Gray colors that are indicative of seasonal water tables were present at this depth The soil permeability of the soil layers at the site was measured using an Aardvark Soil Permeability The Aardvark Permeameter maintains a constant head of water in a bore hole The rate of water movement is measured over time The results are entered into an Excel spreadsheet program that computes the permeability value in inches per hour The Aardvark field forms are attached The test results are presented below The approximate locations of all borings and tests are shown on the accompanying drawing Prior to beginning the tests, the bore hole was allowed to soak for about 15 minutes This is to fill the soil pore spaces around the boring so that saturated hydraulic conductivities are measured Soil attention that the lights out front of the department need looked at Old business- The Boy Scouts want to do some work on the playground so we will just let them do what they can with the equipment we have and not get the things at Camp Lejuene Tony said that the incentive payout is ready for approval The total amount for the year is $3000 00 less than last year He mentioned that the pay out for two officers is higher because they spend a lot more time at the station doing many other things The approved plan does not include extra amounts to officers, but he feels that these 2 have really Community Association Annual Meeting November 10, 2011 The Annual meeting was called to order at 7 02 pm Members preset were Milon, Josie, Paul, Andrew, Melinda, Robert and Tony Thirteen members from the district attended the annual meeting and signed the roster Paul informed all present that sig- nificant strides had been made on building the new fire station and that would be discussed during the meeting Secretary's Report Secretary Josie Mullins read the meeting minutes from October regular monthly meeting and with no dis- cussion, deletions or corrections the minutes were approved as read Treasurers Report Everyone attending had an opportunity to look over the treasurer's report and with no discussion, dele- tions or corrections the report was approved as presented Fire Department Report Tony Matthews gave the fire department's report and reported that in 2011 the fire department had 204 calls 43 fires, 29 wrecks, 7 EMS Assists, 3 Search and Rescue, 6 Hazmat, 23 alarm activations, 20 complaints and 73 can- celed en route He informed the members that to date for 2001 the fire department had 177 calls and counting Tony informed everyone that that the Christmas party was scheduled for Dec 3 and Santa Claus would be on the Creek Dec 17 He spoke about the new computer and that it had not been purchased yet because he wanted to talk with Paul on the computer specs and see if he could get a better deal with the holiday sales approaching With no further discussion, the report was approved as read EMS Report Melinda brought the members up to speed on the EMS She indicated that the call volume was down roughly 15%throughout the county She did not have Broad and Gales Creek EMS call Volume but said it was down and the reasoning was the economy and people not going to seek medical attention She told the members that that EMS was maintaining in this rough economy Building and Grounds Report Milon discussed the new building project and gave the members the opportunity to look over the drawings and the cost projections for the new 5900 sq It proposed engine bays He told the members present that we have added bathrooms and a repair/maintenance shop to the new bays and the estimates cover these additions Milon and Paul told the members that the two cost projects were low and high and did not include the architect, engineering and any change orders that may be made It also does not include a security system for the new building and furnishings Old Business Recognized the Boy Scout leaders that were in attendance The Boy Scouts informed the board that
