Enclosed, please find five (5) copies of the 401/404 application (PCN 'form). A
check for the application review fee is also enclosed. This "After- the - Fact" PCN is to
document and permit the existing driveway culvert and fill material that was installed by
the previous property owner without a permit. A driveway culvert was installed in an un-
named tributary to the Horsepasture, River to provide driveway access to a home site on
Lot 36, Panthertown Trail. The Horsepasture River `is, classified as C; `Trout. A
Jurisdictional Determination (JD) was prepared by Nick Roark, Ecological Associates,
Inc., and submitted to US Army Corps of Engineers in May 2014.
