Construction of the referenced project for the Town of Bakersville is complete and the new WWTP
is ready for operation. The following Professional Engineer's Statement is provided in accordance
with project's Authorization to Construct dated August 2, 2002:
I, J. Vance McGougan, as the duly registered Professional Engineer in the State of North
Carolina, having been authorized to observe weekly, the construction of the Wastewater
Treatment Plant Improvements for the Town of Bakersville project in Mitchell County,
for the Town of Bakersville, state that to the best of my abilities, due care and diligence was
used in the observation of the following installations:
• Submermsible 350 gpm influent pumps
• 0.20 MGD ICEAS aeration treatment system with dual 25 HP 230 V scfm blowers
• UV Disinfection
• Effluent weir and effluent flow metering and sampling
• Conversion of the existing WWTP to sludge anaerobic digestion, thickening, and storage

Town of Bakersville WWTP

The Division of Water Quality's NPDES Unit has reviewed your request for an Authorization to Construct at
the Town of Bakersville Wastewater Treatment Plant. The Division finds the proposed improvements to be
satisfactory and hereby grants authorization for the following:
➢ Submersible 350 gpm influent pumps
➢ 0.2 MGD ICEAS aeration treatment system with dual 25 Hp 230 V scfm blowers
➢ UV disinfection
➢ Effluent weir and effluent flow metering and sampling
➢ Conversion of the existing WWTP to sludge aerobic digestion, thickening, and storage
This Authorization to Construct is issued in accordance with NPDES Permit No. NC0025461, and shall be
subject to revocation unless the wastewater treatment facilities are constructed in accordance with the terms
and conditions of the permit.
The Permittee shall notify the Water Quality Supervisor of the Asheville Regional Office, telephone number
[phone], at least forty-eight (48) hours prior to operation of the installed facilities, so that an in -place
inspection can be made. Notification shall be made during the normal office hours of 8:00 a.m. until 5:00 p.m.,
Monday through Friday.
Engineer's Certification
Upon completion of construction and prior to operation of the modified facility, the Permittee shall provide
the Division with the enclosed certification from a professional engineer registered in North Carolina
certifying that the permitted facility has been installed in accordance with NPDES Permit NC0025461,
Authorization to Construct 025461A01, and the approved plans and specifications. The Certification should
be submitted to: NCDENR / DWQ NPDES Unit, 1617 Mail Service Center, Raleigh, North Carolina 27699-
1617.
Requirements for Certified Operator
The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC)
of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least

equivalent to or greater than the classification assigned to the wastewater treatment facilities by the
Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate
type and grade to comply with the conditions of Title 15A, Chapter 8G, .0200. The ORC of the facility must
visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding
weekends and holidays; and must properly manage and document daily operation and maintenance of the
facility and comply with all other conditions of Title 15A, Chapter 8G, .0200.
Additional Requirements
The Operational Agreement between the Permittee and the Environmental Management Commission is
incorporated herein by reference and is a condition of this Permit. Noncompliance with the terms of the
Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation of
or failure to act in accordance with the terms and conditions of this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee
to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute
143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying with any
and all statutes, rules, regulations, or ordinances which may be imposed by other govemment agencies (local,
state, and federal) which have jurisdiction.
The Permittee shall maintain a copy of the approved plans and specifications on file for the life of the facility.

This request is being made on behalf of the Town of Bakersville in order to receive an
authorization to construct permit for the submitted plans and specifications to increase the
existing permitted wastewater treatment plant from 0.075 MGD to 0.20 MGD.
The project consists of the following:
New influent screening and grit removal,
New submersible 350 gpm influent pumps,
New influent flow splitter box,
New 0.200 mgd ICEAS aeration treatment system with dual 25 HP 230 scfm blowers,
New dual tertiary filtration system,
New UV disinfection system,
New effluent flume and effluent flow metering and sampling,
The existing WWTP will be converted to sludge aerobic digestion, thickening and sludge holding
basins.
This project is in compliance with the requirements of an S.O.C. schedule between the Town of
Bakersville and DWQ. The Asheville Regional Office has been in close coordination with our
office on this project and have been copied directly on this submittal.
