The Division of Water Quality (the Division) hereby issues this final permit for theubjt facility. This permit
is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007, or
as subsequently amended.
Changes to Your Previous Permit. The Division has revised your effluent limits and monitoring
conditions to reflect updated basinwide permitting guideline for the nutrient -sensitive Neuse River. From
your application and correspondence, the Division notes that you have not yet completed upgrades to your
existing WWTP as permitted by Authorization to Construct Permit 025712ACB. Therefore, this renewal
includes no other significant changes to your previous permit.
If any parts, measurement frequencies, or sampling requirements contained in this permit are unacceptable,
you have the right to an adjudicatory hearing upon written request within thirty (30) days after receiving this
letter. Your request must be in the form of a written petition conforming to Chapter 150B of the North
Carolina General Statutes, and must be filed with the office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made, this permit shall be final and
binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division may require
modification, or revocation and re -issuance of this permit. Please notice that this permit does not affect your
legal obligation to obtain other permits required by the Division of Water Quality, the Division of Land
Resources, the Coastal Area Management Act, or other federal or local governments.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge wastewater from a facility located at
Town of Hookerton Wastewater Treatment Plant
Highway 123
North of Hookerton
Greene County
to receiving waters designated as Contentnea Creek in the Neuse River Basin in accordance with
effluent limitations, monitoring requirements, and other conditions set forth in Parts I,11, III and IV
hereof.
This permit shall become effective July 1, 2008.
This permit and authorization to discharge shall expire at midnight on May 31, 2013.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
Town of Hookerton
is hereby authorized to:
1. continue to operate an existing 0.060 MGD wastewater treatment plant consisting of:
• influent bar screen
• three cell lagoon with adjustable effluent baffles and a small

located on Highway 123, North of Hookerton, Greene County and,
2. upon completion of construction, pursuant to Authorization to Construct 025712ACB,
operate a 0.060 MGD Duckweed Treatment System at the same location consisting of:
• influent bar screen
• influent lagoon with pump station at overflow
• Duckweed Treatment System - twelve concrete basins (each 30' x 100'
x 4' deep), supernatant and recycle pumps, duckweed collection
troughs, waste pump, and chiller
• effluent polishing filter
• two -bank UV disinfection system
• effluent / stormwater pump station
• effluent end -pipe diffuser; and,
3. discharge from said treatment works through Outfall 001 at a specified location (see attached
map), into Contentnea Creek, a Class C-Swamp NSW waterbody within the Neuse River
Basin.

A. (1) EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS
Beginning on the effective date of this permit and lasting until completion of construction of the
Duckweed Treatment System or until expiration, whichever is sooner, the Permittee is authorized to
discharge treated wastewater from Outfall 001. Such discharges shall be limited and monitored by the
Permittee as specified below:

respective influent value (i.e., 85% removal required).
3. TRC -- Effluent monitoring and limit apply only if chlorine or a chlorine derivative is added. Because low-
level TRC is currently difficult to quantify in a wastewater matrix, the Division shall consider all effluent
TRC values reported below 50 AWL to be "in compliance" with the permit. However, the Permittee shall
continue to record and submit all values reported using North Carolina -certified field and lab test methods,
even if these. values fall below 50 µg/L.
4. For a given wastewater sample, TN = TKN + NO3 N + NO2-N, where TN is total nitrogen, TKN is total
Kjeldalil Nitrogen, and NO3-N and NO2-N are nitrate and nitrite nitrogen, respectively.
5. TN load is the mass quantity of total nitrogen discharged in a given time period. See condition A. (4) of this
permit.
The Permittee shall discharge no floating solids or foam visible in other than trace amounts.

Beginning upon completion of construction of the Duckweed Treatment System and lasting until
expiration, the Permittee is authorized to discharge treated wastewater from Outfall 001. Such
discharges shall be limited and monitored by the Permittee as specified below:

A. (4) CALCULATION OF TOTAL NITROGEN LOADS
a. The Permittee shall calculate monthly and annual TN Loads as follows:
i. Monthly TN Load (lb/mo) = TN .x TMF x 8.34
where:

composite samples collected during the month
= the Total Monthly Flow of wastewater discharged during the
month (MG/mo)
8.34 = conversion factor, from (mg/L x MG) to pounds
ii. Annual TN Load (lb/yr) = Sum of the 12 Monthly TN Loads for the calendar year
b. The Permittee shall report monthly Total Nitrogen results (mg/L and lb/mo) in the discharge
monitoring report for that month and shall report each year's annual results (lb/yr) in the December
report for that year.
