Dear Ms. Purser:
The Division of Water Quality's NPDES Unit has reviewed the Town of Hookerton's request for
an Authorization to Construct a nutrient removal system at the Hookerton WWTP. The Division
finds the proposed improvements and design to be satisfactory and hereby grants authorization
for construction and installation of the following:
• a duckweed treatment system, consisting of
• 12 - 30'x100'x4' deep concrete basins,
• greenhouse covers and spray cooling system for each basin,
• supernatant and recycle pumps, and
• duckweed collection troughs, waste pumps, and chiller;
• effluent polishing filter;
• two -bank UV disinfection system;
• effluent/ stormwater pump station;
• influent pump station at the overflow from Lagoon 1; and
• all related earthwork, structures, equipment, piping, controls, and other appurtenances, as
presented in the approved plans and specifications.
Upon completion of these improvements and submittal and acceptance of the Engineer's
Certification (see below), the approved design capacity of the treatment facilities shall remain at
0.060 MGD.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES
Permit No. NC0025712, issued October 9, 2000, and shall be subject to revocation unless the
wastewater treatment facilities are constructed in accordance with the conditions and limitations
specified in that permit.
Special Provisions
Reliability - Standby Power. The state's water quality rules (T15A NCAC 02H. 0214) require
permittees to ensure that power failures do not disable treatment and disinfection facilities and
render the receiving waters unsafe. To meet this requirement, permittees can provide dual -feed

power (connections to two different substations); install standby power (such as an on -site
generator); or demonstrate that waters would not be impacted by a power failure (emergency
storage).
This Authorization is conditioned upon the Town meeting the above requirement prior to
operation of the new treatment system. Providing a dual -feed or a standby power supply are
both feasible options. However, the Town already has emergency capacity in its three existing
lagoons. The Town should verify that the available storage capacity is sufficient. In addition to
using the available capacity in Lagoon 1, it may be to the Town's advantage to maintain the
ability to use Lagoons 2 and 3 for emergency storage.
Facility Classification. Implementation of the plant improvements will prompt its
reclassification to a Grade II facility. This, in turn, will require that the Operator in Responsible
Charge (ORC) be certified as a Grade II operator and that the monitoring frequencies in the
Town's NPDES permit be modified to reflect the new status.
Solids Management. The Solid Waste Management Plan submitted with the final design
documents is acceptable. The Division understands this is meant to be an interim disposal option
until the Town identifies a more beneficial alternative. Before implementing any other
alternative for solids disposal or reuse, the Town must submit a revised plan for Division
approval.
Performance. In the event that the facilities fail to perform satisfactorily, including the creation
of nuisance conditions, the Permittee shall take immediate corrective action, including those as
may be required by this Division, such as the construction of additional or replacement
wastewater treatment or disposal facilities.
Notification Requirements
The Permittee shall notify the Washington Regional Office, telephone number (252) 946-6481,
at least forty-eight (48) hours in advance of operation of the installed facilities, so that an in -
place inspection can be made. Such notification to the regional supervisor shall be made during
the normal office hours of 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Engineering Certification
Upon completion of construction and prior to operation of the modified facility, the Permittee
shall provide the Division with a certification from a professional engineer certifying that the
permitted facility has been installed in accordance with the NPDES Permit, this Authorization to
Construct, and the approved plans and specifications. Mail the Certification to the NPDES Unit,

ORC Requirements
The Permittee shall employ a certified wastewater treatment plant operator to be in responsible
charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the
type and grade at least equivalent to or greater than the classification assigned to the wastewater
treatment facilities by the Certification Commission. The Permittee must also employ a certified
back-up operator of the appropriate type and grade to comply with the conditions of Title 15A,
Chapter 8G, .0200. The ORC of the facility must visit each Class I facility at least weekly and

each Class II, III, and IV facility at least daily, excluding weekends and holidays; and must
properly manage and document daily operation and maintenance of the facility and comply with
all other conditions of Title 15A, Chapter 8G, .0200.
Additional Conditions
The Operational Agreement between the Permittee and the Environmental Management
Commission is incorporated herein by reference and is a condition of this Permit.
Noncompliance with the terms of the Operational Agreement shall subject the Permittee to all
sanctions provided by G. S. 143-215.6 for violation of or failure to act in accordance with the
terms and conditions of this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other
government agencies (local, state, and federal) which have jurisdiction.
The Permittee shall maintain a copy of the approved plans and specifications on file for the life
of the facility. One (1) copy of the approved plans and specifications is enclosed for your
records.
