This permit was renewed in August 2019, and the attached letter was sent out. However, no one requested a copy of
the renewal COC, and so none was ever generated for the permit records.
I have temporarily reactivated the original COC request form so you can request a copy of the COC from that
time. Please note that all information will populate as is it would have in 2019. If there have been updates to name or
ownership since then, the COC will not reflect them.
