Director NORTH CAROLINA Quality August 19, 2022 Jerry Howerton Maintenance Manager NC Department of Natural and Cultural Resources 13304 Creedmoor road Wake Forest, NC 27587 Subject: Compliance Evaluation Inspection Permit # WQ0005247 Facility: Falls Lake SRA - Rollingview WWTF/WW Irrigation system Durham County Dear Mr. Mumford: On August 16,2022 Jim Westcott from the Raleigh Regional Office visited your wastewater treatment and irrigation system and met with David Mumford, Vincent Shea, and Joel Valentine to evaluate compliance with the subject permit. Your collective cooperation and comprehensive tour of your facility was informative and greatly appreciated. You are permitted to operate a 9,999 gallon per day (GPD) wastewater collection, treatment and irrigation facility consisting of ten influent pump stations at the Rolling view park; a grit chamber and piping to connect to the existing shower at Pump station 2; a septic tank; a pump station with two 22 gallon per minute (GPM) effluent pumps and high water alarms; approximately 500 linear feet (LF) of 2-inch force main to discharge collected wastewater from the Superintendents' residence to the existing wastewater facilities; approximately 345 linear feet (LF) of 8-inch gravity sewer; approximately 225 LF of 6-inch gravity sewer; a 2 horsepower (hp) pump station with duplex pumps and high water alarms; eight 5 hp pump stations each with duplex pumps and high water alarms (as upgraded); two 7.5 hp booster pump stations (as upgraded) each with duplex pumps and high water alarms; approximately 17,650 LF of 2,3 and 4-inch force main; a 7 million gallon (MG) stabilization lagoon; a tablet chlorinator; a 3200 gallon chlorine contact chamber; a 150 GPM irrigation pump station with dual submersible pumps; two 3.55 acre irrigation zones; and all associated piping, valves, controls and appurtenances with no discharge of wastes to surface waters. Section I (1) of the subject permit requires the permittee to request renewal of the permit no later than six months prior to the expiration of the permit on official division forms. At the time of inspection, the permittee presented a receipt of acknowledgement of the application renewal letter by the division dated July 25, 2022. Please ensure that future renewal applications are completed and submitted no later than six months prior to expiration to the division as required. At the time of inspection, required logbook, maintenance, flow, calibration records and annual standard soil fertility analysis was available for review. Each lagoon freeboard level was clearly marked, identified, and recorded weekly, including discharge pump run hours and spray application rate(s). The treatment lagoon was secured and proper signage on the perimeter fencing. The two 3.55-acre spray field(s) were in good condition with consistent vegetative cover in each zone. The schedule of application to each of the spray fields is recorded each day to ensure even distribution. Spray nozzles are inspected for proper operation are the facility grounds were clean and structured. Each spray field/zone discharge nozzle(s), piping connections were inspected while on site. A section of the aluminum discharge line on the east side entrance of the spray field had become dislodged from a tree branch and will need to be repaired. The section was secured at the main line with a valve cap to ensure the area is receiving adequate pressure and an even spray -distribution pattern in each zone. 1. OPERATION AND MAINTENANCE REQUIREMENTS: Section I (1) of the permit Monitoring and Reporting requirements state that the facilities shall be properly maintained and always operated. The facilities shall be properly maintained and always operated. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. The permittee shall maintain an Operation and Maintenance Plan, which at a minimum shall include operations functions, maintenance schedules, safety measures and a spill response plan. [15A NCAC 02T .0507] Within 30-days of receiving this letter, please forward a document showing the repaired line and the inspection schedule for the spray field to this office. 2. MONITORING AND REPORTING REQUIREMENTS: Records were not available for the requirements of section 8 [15A NCAC 02T .0108(b)(1)] of the permit regarding residuals disposal. No material has been removed from the site. Records indicating the sludge level in the lagoon were not available at the time of inspection although the previous estimate was two years ago. Within 30-days of receiving this letter, please forward a plan for evaluating the solids levels in the lagoon to this office. The non -discharge monitoring reports (NDMR's) for the subject facility were reviewed for the period January 2021 July 2022, and the following observations were detected [see attached Monitoring Report (MR) Violations Report for details]; (a) Monitoring Violations — The facility reported the following limit exceedances during the 2-year review period: Two CL2 residual and two pH. (b) Reporting Violations — Four Total Nitrogen as N The wastewater treatment system should be periodically inspected to ensure the treatment components are always maintained and in good operating order. You are also reminded to maintain all monitoring data and associated maintenance records onsite for a minimum of three years and available for inspection. Within 30-days receipt of this letter, please submit a written response to this office indicating the actions you will take or have taken to comply with or resolve the issues noted in items 1 and 2 above. The office will address the monitoring report violations under a separate cover.
