Division personnel have reviewed and approved your application for
renewal of the subject permit. Accordingly, we are forwarding the attached
NPDES discharge permit. This permit is issued pursuant to the requirements
of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection
Agency dated May 9, 1994 (or as subsequently amended.)
The permit authorizes the City of Hickory to discharge up to 0.225 MGD
of treated wastewater to Lyle Creek, a class WS-IV CA water in the Catawba
River Basin. The draft permit includes discharge requirements and/or
limitations for flow, biochemical oxygen demand (BOD), total suspended solids
(TSS), ammonia, fecal coliform bacteria, total residual chlorine, total nitrogen,
total phosphorus, copper, zinc, and chronic toxicity.
The following modifications noted in the draft permit, remain the same:
• Quarterly effluent monitoring for cyanide and phenols has been
deleted. A review of monitoring data indicated that there is no
reasonable potential for these parameters to exceed the water quality
standard and are no longer parameters of concern.
• Monthly monitoring for copper and zinc has been added to permit,
instead of the previous quarterly monitoring. A review of submitted
monitoring data indicated that there is reasonable potential for these
parameters to exceed the water quality standard.

• A total residual chlorine limit of 28 ug/1 has been added to reflect the
Division's current policy for protection against chlorine toxicity
instream. The limit for total residual chlorine shall become effective
upon completion of the installation of a disinfection system but no
later than 18 months from permit issuance (February 1, 2007). If a
method different than chlorination/dechlorination is used, the total
residual chlorine limit will not be applicable.
If any parts, measurement frequencies or sampling requirements
contained in this permit are unacceptable to you, you have the right to an
adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed
with the office of Administrative Hearings, 6714 Mail Service Center, Raleigh,
North Carolina 27699-6714. Unless such a demand is made, this permit shall
be final and binding.
Please take notice that this permit is not transferable. The Division may
require modification or revocation and reissuance of the permit. This permit
does not affect the legal requirements to obtain other permits, which may be
required by the Division of Water Quality, or permits required by the Division of
Land Resources, Coastal Area Management Act, or any other Federal or Local
governmental permits may be required.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked, and as of this issuance, any previously issued permit bearing this number
is no longer effective. Therefore, the exclusive authority to operate and discharge from this
facility arises under the permit conditions, requirements, terms, and provisions included
herein.

1. Continue to operate an existing 0.225 MGD wastewater treatment facility with the
following components:
• Dual bar screens
• Two aeration basins
• Two secondary clarifiers
• Chlorine contact basin (gas disinfection)

The facility is located at Hickory -Catawba WWTP, Sixth Avenue NW, north of
Catawba, Catawba County.

SUPPLEMENT TO EFFLUENT LIMITATIONS
AND MONITORING REQUIREMENTS
SPECIAL CONDITIONS
A. (2.) CHRONIC TOXICITY PERMIT LIMIT (QRTRLY)
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to
Ceriodaphnia dubia at an effluent concentration of 2.1%.
The permit holder shall perform at a minimum, quarteriu monitoring using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or
"North Carolina Phase II Chronic Whole Effluent Toxicity T t-ProcPriure" (Revised=ebruary 1998) or subsequent
versions. The tests will be performed during the months erand Decembffluent

If the test procedure performed as the first test of any single quarter re ults in a failure or ChV below the permit
limit, then multiple -concentration testing shall be performed at a minimum, in each of the two following months
as described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998)
or subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the highest
concentration having no detectable impairment of reproduction or survival and the lowest concentration that does
have a detectable impairment of reproduction or survival. The definition of "detectable impairment," collection
methods, exposure regimes, and further statistical methods are specified in the "North Carolina Phase II Chronic
Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring, Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B for the
pass/fail results and THP3B for the Chronic Value. Additionally, DWQ Form AT-3 (original) is to be sent to the
following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Branch no later than 30
days after the end of the reporting period for which the report is made.
Test data shall be complete, accurate, include all supporting chemical/physical measurements and all
concentration/response data, and be certified by laboratory supervisor and ORC or approved designate signature.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the
facility name, permit number, pipe number, county, and the month/year of the report with the notation of "No
Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Branch at
the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be
required during the following month. Should any test data from this monitoring requirement or tests performed
by the North Carolina Division of Water Quality indicate potential impacts to the receiving stream, this permit
may be re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism
survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an
invalid test and will require immediate follow-up testing to be completed no later than the last day of the month
following the month of the initial monitoring.

SUMMARY OF FACILITY INFORMATION AND WASTELOAD ALLOCATION
The City of Hickory is requesting renewal of the former Catawba WWTP. Hickory purchased the
Town of Catawba WWTP in July 2003. The WWTP discharges into Lyle Creek, a class WS-IV CA
stream, in CTB32 subbasin. The facility does not have a pretreatment program. Its wastewater
is primarily domestic.
Lyle Creek is not listed on North Carolina's 2002 303(d) list.
There is a management strategy for the Lyle Creek watershed. Results of a 1988 modeling
analysis indicated that BOD5 and NH3 had been over allocated in Lyle Creek under prior
wasteload allocation procedures. The model was done to establish NPDES permit limits for
the Town of Conover WWTP (discharging upstream of Catawba) to protect the DO standard and
was used to guide recommendation for future expansions and new facilities in the Lyle Creek
watershed. Expanding facilities in the Lyle Creek watershed will receive BOD and NH3 limits
that will hold their load of oxygen consuming wastes constant. All new facilities discharging to
the Lyle Creek watershed will receive limits for oxygen consuming waste of ultimate summer
BOD not greater than 21 mg/1 (equivalent to 8 mg/1 BOD5 and 2 mg/1 NH3 for domestic
discharges.)
In the 2004 Catawba River Basinwide Management Plan --The Aquatic Life use support rating
for 6.3 miles of Lyle Creek from Bakers Creek to US Highways 64 and 70 is supporting based
on monitoring. No problem parameters are listed in this report. Previously sediment was listed
as a problem parameter in this segment of Lyle Creek.
The 1997 benthos rating at US Highways 64 and 70 was Good -Fair. The results of 1997 fish
sampling at US 79 was Good.

COMPLIANCE SUMMARY:
Facility appears to be well operated. Only one violation report in Compliance System, pH
violation in March 2005. All other limits have been met.
INSTREAM MONITORING REQUIREMENTS:
Instream monitoring is not required for this WWTP.
REASONABLE POTENTIAL ANALYSIS
The following metal/toxicity parameters are monitored in the NPDES permit: Copper, cyanide,
phenols, and zinc.
Data for these parameters was reported in the discharge monitoring reports (DMR) and a RPA was
done:
Reasonable potential analysis was conducted based on data from 2002 through April 2005. Results
and data analysis are attached.
• Monthly monitoring should be started for copper based on RPA. Copper results had
fifteen (15) reported values. There was reasonable potential to exceed the allowable acute
concentration but not the allowable chronic concentration for the NC action level.
Monthly monitoring is required based on the class of the facility.
• Monthly monitoring should be started for zinc based on RPA. Zinc results had fifteen (15)
reported values. There was reasonable potential to exceed the allowable acute and
chronic concentrations for the . NC action level. Monthly monitoring is required based on
the class of the facility.
• Quarterly monitoring for cyanide and phenols should be deleted. Based on analysis,
neither parameter showed reasonable potential to exceed allowable concentrations.
PROPOSED CHANGES:
The following modifications have been made to the permit:
• The addition of a Total Residual Chlorine limit of 28 ug/1 based on DWQ procedure
that requires limit for all municipal discharges. Eighteen month compliance schedule
will be given_
• Quarterly monitoring for copper and zinc will be changed to monthly monitoring
based on the results of RP analysis.
• Quarterly monitoring for cyanide and phenols will be deleted based on the results of
the RP analysis.

I want to give you all an update on information coming out of the Duke/FERC Reliscensing process on the Catawba and find out where we are
on permit renewals in the Rhodhiss area.
We are starting to get preliminary data from the water quality models Duke is developing for the Catawba mainstem. Some of that data is
included in the draft study report - link below. Not currently included in the report is evidence demonstrating the impact the Valdese WWTP
is having on Lake Hickory. This is particularly interesting because the Valdese discharge is into Lake Rhodhiss, about 6 miles upstream of the
dam. Our working assumption has been Valdese is a major contributor to the eutrophic conditions that led to Rhodhiss' impairment. The
model, however, is indicating the Valdese discharge has a larger impact on Lake Hickory. (Please recall that Lake Hickory is on the
borderline of nutrient impairment.) The majority of Rhodhiss' point source problems appear to be coming from Morganton and Lenoir's
WWTPs. The model will be able to isolate Morganton's impact on Rhodhiss, but unfortunately, the Lenoir discharge is hidden in the total
load entering from Lower creek. •
Have these WWTP permits been renewed yet? If so, what were their terms? If not, can we set aside some time to discuss optimization options
and the like?

SUMMARY OF FACILITY INFORMATION AND WASTELOAD ALLOCATION
The City of Hickory is requesting renewal of the former Catawba WWTP. Hickory purchased the
Town of Catawba WWTP in July 2003. The WWTP discharges into Lyle Creek, a class WS-IV CA
stream, in CTB32 subbasin. The facility does not have a pretreatment program. Its wastewater
is primarily domestic.
Lyle Creek is not listed on North Carolina's 2002 303(d) list.
There is a management strategy for the Lyle Creek watershed. Results of a 1988 modeling
analysis indicated that BOD5 and NH3 had been over allocated in Lyle Creek under prior
wasteload allocation procedures. The model was done to establish NPDES permit limits for
the Town of Conover WWTP (discharging upstream of Catawba) to protect the DO standard and
was used to guide recommendation for future expansions and new facilities in the Lyle Creek
watershed. Expanding facilities in the Lyle Creek watershed will receive BOD and NH3 limits
that will hold their load of oxygen consuming wastes constant. All new facilities discharging to
the Lyle Creek watershed will receive limits for oxygen consuming waste of ultimate summer
BOD not greater than 21 mg/1 (equivalent to 8 mg/1 BOD5 and 2 mg/1 NH3 for domestic
discharges.)
In the 2004 Catawba River Basinwide Management Plan --The Aquatic Life use support rating
for 6.3 miles of Lyle Creek from Bakers Creek to US Highways 64 and 70 is supporting based
on monitoring. No problem parameters are listed in this report. Previously sediment was listed
as a problem parameter in this segment of Lyle Creek.
The 1997 benthos rating at US Highways 64 and 70 was Good -Fair. The results of 1997 fish
sampling at US 79 was Good.

COMPLIANCE SUMMARY:
Facility appears to be well operated. Only one violation report in Compliance System, pH
violation in March 2005. All other limits have been met.
INSTREAM MONITORING REQUIREMENTS:
Instream monitoring is not required for this WWTP.
REASONABLE POTENTIAL ANALYSIS
The following metal/toxicity parameters are monitored in the NPDES permit: Copper, cyanide,
phenols, and zinc.
Data for these parameters was reported in the discharge monitoring reports (DMR) and a RPA was
done:
Reasonable potential analysis was conducted based on data from 2002 through April 2005. Results
and data analysis are attached.
• Monthly monitoring should be started for copper based on RPA. Copper results had
fifteen (15) reported values. There was reasonable potential to exceed the allowable acute
concentration but not the allowable chronic concentration for the NC action level.
Monthly monitoring is required based on the class of the facility.
• Monthly monitoring should be started for zinc based on RPA. Zinc results had fifteen (15)
reported values. There was reasonable potential to exceed the allowable acute and
chronic concentrations for the . NC action level. Monthly monitoring is required based on
the class of the facility.
• Quarterly monitoring for cyanide and phenols should be deleted. Based on analysis,
neither parameter showed reasonable potential to exceed allowable concentrations.
PROPOSED CHANGES:
The following modifications have been made to the permit:
• The addition of a Total Residual Chlorine limit of 28 ug/1 based on DWQ procedure
that requires limit for all municipal discharges. Eighteen month compliance schedule
will be given_
• Quarterly monitoring for copper and zinc will be changed to monthly monitoring
based on the results of RP analysis.
• Quarterly monitoring for cyanide and phenols will be deleted based on the results of
the RP analysis.

have taken place at the facility. Pending review and approval by the P&E, it is recommended that
the permit be renewed as requested.

Dear Mrs. Stephens:
Attached please find the completed renewal application package including Sections A, B, C, E,
Sludge Management Plan Narrative, Site Topographic Map and Process Flow Diagram for the
above referenced site. This application is requesting renewal of the existing permit with no
capacity modifications having taken place at the facility with the exception of ownership and
sludge disposal methods.
The City of Hickory has upgraded various components of this operation since purchasing in July
2003. The influent and effluent flow measuring devices have been replaced, updated lab
equipment has been purchased and instituted and a new chlorine room has been constructed.

Biosolids Management Plan for Hickory -Catawba Wastewater Treatment Plant

To Whom It May Concern:
The City of Hickory owns and operates three wastewater treatment facilities: Hickory -Catawba
WWTP (NPDES # NC 0025542), Northeast WWTP (NPDES # NC 0020401) and Henry Fork
WWTP (NPDES # NC 0040797). All city owned wastewater facilities transport biosolids
generated via road tractor and tanker to the Hickory Regional Compost Facility (WQ 0004563).
The Hickory Regional Compost Facility then commingles and processes to convert all biosolids to
a Class "A" compost marketed as "Catawba ORGRO".
The Hickory Regional Compost Facility has operated for over eight (8) years without a significant
shut down. In the unlikely event of a shut down at this facility, the City of Hickory has an
agreement with the Catawba County Landfill which allows Hickory to thicken biosolids to 9.0%
then dispose of directly in land fill cells. This alternative is to meet the emergency backup
operation requirement.
