ADDITIONAL REGIONAL STAFF REVIEW COMMENTS/ATTACHMENTS (Optional /If Needed) This site is associated with IHSB incident number NONCD0002787. George Adams of IHSB is PM over this facility. During the site visit, DWR staff observed the proposed injection well network. The injection network of Seventy-three (73) 1-inch injection points located inside the building and on the Eastern portion of the property. Forty (40) additional injection points are to be installed around the area of the former WWT basin. The existing monitor wells installed for GW sample collection to monitor the daughter product degradation during post -injection monitoring are sufficient. Near -surface lithology is primarily silty sand and silt. This sediment type is not likely to have a high transmissivity through a tight pore space. There is a perennial surface water feature named Muddy Fork, located to the east of the subject site. Monitor wells 5D and 5S are located 200 feet from the SE corner of the property boundary. Muddy Fork is located 200 yards from the location of wells 5D&S. Mention was made during the site visit that if CVOC concentrations were elevated in wells MW-5D and MW- 5S, surface water samples should be collected to see if constituents of concern were daylighting into Muddy Fork. Muddy Fork runs through a pre -regulatory landfill. There are GW monitoring wells surrounding the landfill. Discarded tires were observed in the landfill. This permit is projecting the possibility of secondary injection if required to complete the remediation of the chlorinated solvents detected during the previous investigation event. If daylighting of constituents has occurred, SW sample locations will be added to the post -injection monitoring plan. The chain link fencing surrounding the facility provides sufficient security around the facility for the injection operations to be contained and controlled.
