Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16& SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter
El Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
El Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions,attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
El If the Applicant Type in Section I.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant
is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a..franchise has been received and that the service area iscontiguous to an existing franchised area.

as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T
.0106(b).

Section III Restrictions on Budget Officer
Interfund and interdepartmental transfer of moneys, except as noted below, shall be accomplished
only with specific advanced approval of the Oxford City Board of Commissioners.
Special Authorizations of the Budget Officer
The Budget Officer shall be authorized to reallocate any appropriation within departments.
The Budget Officer shall be authorized to execute interfund and interdepartmental transfers in
emergency situations. Notification of all such transfers shall be made to the Board of

Our customer notification plans involves contacting the local radio and television stations
to put out public service announcement. We could also send out police cars and other
emergency vehicles to make neighborhood announcements.

Pump Station First Responder
1. DO YOU HAVE POWER AT THE STATION? IF NOT AND THE GENERATOR IS NOT RUNNING
START IT MANUALLY.
2. YOU HAVE POWER BUT THE PUMP HAS FAILED TO START,TRY AND RESET THE PUMPS.
3. IF THE PUMPS WILL NOT RESET CHECK THE FLOATS AT(COON CREEK, ROLLINGWOOD,
OXFORD PARK)AND PUMP TRANSDUCERS AT ALL OTHER STATIONS.
4. CALL THE PUMP MECHANIC OR OTHER BACK UP.
5. GORMAN-RUPP STATIONS CHECK TO SEE IF THE PUMPS ARE HOT THEY MAY HAVE LOST
PRIME.
6. REMEMBER ALL OUR STATIONS WILL RUN ON ONE PUMP.
7. IF YOU ARE UNABLE TO START ANY PUMPS CALL EMERGENCY CONTRACTORS FROM THE LIST
IN EMERGENCY RESPONSE PLAN OR ON CALL PHONE.
8. ALL STATIONS EXCEPT COON CREEK CAN BE KEPT ON LINE USING LOCAL SEPTIC TANK TRUCKS
TO HAUL.
9. WE HAVE A 6" PUMP AT WASTE WATER TREATMENT PLANT TO BYPASS PUMP IF NEEDEED.

SPILL RESPONSE PLAN
EMERGENCY RESPONSE TIME:
The first responder shall be on job site no later than 30 minutes after receiving first notification.
ASSESSMENT
The first person to respond will assess the situation, and call for back-up immediately.
(If the spill is in a creek, call everyone on the crew.)
Take notes: What time did you arrive?
What time did help arrive?
What type of spill (manhole, pump station, did it reach a creek.)?
Actions to be taken:
Contain the spill.
If the spill reaches a creek:
1. Go down stream until there is no evidence of sewer in the creek.
2. Dam the creek using straw to contain the solids.
3. Use the backhoe to build a dam of sand or dirt.
4. Have truck 10 start to unstop the line using the flusher.
5. Have truck 6 or 8 set up a pump and start to pump everything back into the nearest manhole.
6. Have someone go up stream turn on a hydrant to help flush the creek.
If the spill is in a parking lot yard or some other area away from surface waters:
1. Contain the spill using whatever method best suits the situation.
2. Use the flusher to unstop the line.
If the spill is at the pump station:
1. Make sure that all pumps are working properly.
2. Call the pump mechanic(Lawrence Skinner 984-514-2359)
3. Contain the spill.
4. You may need to call an outside contractor to pump the station.

After all service is returned to normal:
1. Clean the area thoroughly, remove and haul away any solids.
2. Lime the area if it will not be washed into a water way.

The first responder is in charge of getting all personal and equipment on the job site or on the way to
the site. He can't shift the responsibility to anyone else or wait until a supervisor is on site. Response
time is very important on a sewer spill. Assess the problem and respond. There is no such thing as over
kill when sewer is on the ground.
Always remember if you have not written it down, it has not been done.

Contingency Plan updated October 2021
1. Do you have power at the station?
> If not and the generator is not running,start it manually.
2. If you have power but the pump has fail,try to reset the pumps.
> If the pump will not reset,check the transducer(CertainTeed, Knots Grove, 1-85,
Mid Way, Rest Area).All other stations (Coon Creek,Willows, Rolling wood,
Kingsbury,Clover Lane)check the floats.
3. Call the pump mechanic or other back up.
4. Gorman-Rupp Stations—Check to see if the pumps are hot. They may have lost prime.
5. Remember all of our stations will operate on one pump.
6. If you are unable to start any pumps, use your pocket card to call outside contractors,
7. All stations except Coon Creek can be kept on line using local septic tank trucks.
8. We have a 6" Diesel powered pump located at the treatment plant that can be used to bypass
pump.
