Based on the information you submitted on 6/30/2022, my site visit on 5/23/2019, and my emails concerning use of
non -reporting Nationwide Permit (NWP) 3 on 7/29 and 11/12/2021, the above referenced project appears to be
authorized under Nationwide Permit (NWP) 3 and does not require notification to the Corps. Your project is described
as follows:
The replacement of NCDOT Bridge 584 on SR 3412 (Tabernacle Church Road) involves the permanent discharge of fill
material into 5 linear feet (<0.01 acre) of stream channel (not a loss of waters) and 0.02 acre of wetlands, and the
temporary discharge of fill material into 10 linear feet (<0.01 acre) of stream channel, necessary to replace and existing
bridge on the some alignment. This project also requires hand clearing (not permanently maintained) in 0.04 acre of
wetlands.
Attached is a copy of the NWP 3 terms and conditions. Please note that the project must be accomplished in strict
accordance with these conditions and your submitted description, and any violation of the conditions or deviation from
your submitted description may subject the permittee to a stop work order, a restoration order and/or appropriate legal
action. The Action Identification Number for this project is SAW-2019-00875.
Please let me know if you have any questions.
Sincerely,
Dave Bailey
