The project manager, Joe Couch and other DOT staff I met out there provided descriptions of what happened and the course of action they took to minimize the impact. I did not observe any remaining excess concrete or residue during the review. It all looked ok to me. Our staff discussed this incident internally to benchmark how others may have handled a situation like this in the past. One point that was made was that DOTs activities are not only covered by DWRs Certifications but also through DOTs NPDES Stormwater Permit NCG010000; specifically in this case Section IIB(1)(e)(i and ii). So while a permit was not needed for this bridge replacement, DOT must manage activities on site such that water quality standards are not violated. In addition, there are requirements for inspections, reporting and record keeping. It was also suggested that when accidents such as this happen, DOT needs to determine exactly what caused the accident (form failure in this case) so that measures can be taken to prevent a similar accident from occurring in the future. We require no additional information at this time. Dave W Heath Slaughter wrote on April 24, 2014: Andy/Dave, ee advised that we've had about a 2.5-3 yard concrete spill into a UT to South Deep Creek on the subject bridge maintenance project. There is no permit as one was not needed. This bridge had timber piers on top of a tall concrete footer (well out of the water). Some of these timber components were rotting so the timbers were replaced with steel and our crew was attempting to encase the new steel and end bent in concrete. This would of course not increase the footprint of the bridge. As you can see in the attached picture, one of the forms failed and concrete was spilled. Our forces responded immediately with the materials that they had on site and began clean-up operations immediately. Whatever damage to aquatic life there may have been is very likely over at this point. We apologize for the unexpected problem. If you need additional information, please let me know.
