Enclosed is additional information requested in our phone conversation August 2, 2006, and
September 10, 2007. According to GC 3404 Item No. 3 application to and payment is not
required for construction of a driveway to asingle-family lot if impacts are less than 25'. The
attached-revised Site Plan shows the approximate location of the proposed driveway and lot.
Proposed impacts are now minimised to 201inear feet of 36" CMP to allow driveway access
to the Subject Property (see attached Site Plan). If you have any questions or concerns do not
