This modification documents the change in name/ownership at the subject facility. All other terms and conditions in the original permit remain unchanged and in full effect. This permit modification is issued under the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency dated December 6, 1983. If any parts, measurement frequencies or sampling requirements contained in this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding. This permit does not affect the legal requirement to obtain other permits which may be required by the Division of Water Quality, the Division of Land Resources, Coastal Area Management Act, or any other Federal or Local government permit that may be required. It is very helpful for us to meet the people we will be working with and to begin to build an understanding of the state's concerns and expectations. As we mentioned during the meeting, our goal is to provide quality cost efficient service to our customers while complying or exceeding state, federal, and local tegulations. Therefore, please accept this letter as my formal request, as an officer of AquaSource, Inc., that the records for all facilities listed on Attachment A be updated and revised to reflect ownership by AquaSource, Inc. In addition, that our Regional Manager, Glenn LaBrecque, be designated as the responsible party. Mr. Leonard Stogner who as a former owner of Rayco Utilities, Inc. and Willowbrook Utilities, Inc. also requests this change and further acknowledges that all matters described and discussed herein are accurate also signs this letter. Mr. Stogner now works as an employee of AquaSource in a role where he is closely involved in the day to day operations but no longer serves in a capacity as a responsible party. He now reports directly to our Regional Manager, Glenn LaBrecque. We believe Mr. Stogner will provide valuable assistance in the position of industrial and construction manager. His knowledge of operations will be utilized in coordinating the efforts of our operators. We hope that your staff and particularly the regional staff will understand that it is optional whom they choose to contact. However, we would hope that they would continue to contact Mr. Stogner or others within the previous Rayco staff for the recurring and routine interactions that are a course of normal day to day operations such as to coordinate an inspection, discuss a minor matter, or direct questions. If for any reason your staff prefers, they may always directly contact Regional Manger Glenn LaBrecque, or me. We would expect that if there are any significant problems or concerns, any Notices of Violation, any enforcement actions, or any other "official" communications, those would be addressed dire- ey to the Regional Manager, Glenn LaBrecque.• : • 2. t In conclusion, let me again thank you and your staff for the courtesy extended to us during our meeting.
