FACILITY OVERVIEW:
This facility is 100% domestic waste and serves the Town of White Lake. The WWTP is an aerated facultative
lagoon system that provides treatment for a service population of around 845 people. The facility has a
permitted and as built flow rate of 0.8 MGD and an average daily flow of 0.46 MGD. The treatment system
includes the following wastewater treatment components:
• Bar screen
• Vortex grit removal chamber
• 4.4-acre aerated multicellular lagoon
• Effluent meter
• Floating aerator units
• Chlorine/dechlorination contact chamber

APPLICATION
• I/I shown as 2 9,0 0 gpd average daily volume
• BOD and TSS design removal - 85%
• Application not signed but contacted and they sent the signed page in separately. Added to the file and
updated in Laserfiche.
• The facility is also tied to Collection System Permit WQCS00136.
• WWTP serves a population of 845 people, past application info mentions tourist population as well.
• Maximum daily flow rates are noted as around 1.2 MGD over last year. Had been as high as 2 MGD two
years ago.
• The application indicates no Significant Industrial Users (SIUs).
Previous app noted engineering study to look at collection system to identify defects and make
recommendations, but no info on the study provided. There has been some construction and repair as noted in
the last inspection report.
INSPECTION
The most recent inspection (07/30/2020) found the facility was in compliance and noted records and
information generally in good condition. The inspection report indicated ongoing construction at the facility to
install a new bar screen and grit removal system and to repair the walls of the chlorine contact chamber.
Recent correspondence with staff from White Lake indicate that these construction activities have been
completed. The inspection report also noted general concerns that housekeeping efforts needed to improve at
the facility. Specifically, with regard to high grass and weed growth around the lagoon and headworks.
COMPLIANCE:
During the permit cycle this facility had two (2) NODs, and one (1) NOV. The NODS were for monitoring
frequency violations for DO and Temperature. The NOV was for a weekly average violation of the fecal coliform
limit. This NOV led to the one (1) enforcement action for this facility over the permit cycle. The total amount
assessed in this enforcement was $ 378.28 with $250 in penalties. The facility also had one monthly average
flow limit exceedance, but this was attributed to impacts of Hurricane Florence.
The Town has collection system permit WQCS00136. There were no incidents or violations associated with this
permit over the past five years and the most recent inspection (June 2019) found the system in compliance.

The data reviewed for the renewal is from April 2017 through January 2022.
REVIEW OF PERMIT LIMITS AND CONDITIONS:
Flow - The facility has had only one (1) Monthly Average flow number exceed the permitted flow of 0.8 MGD
over the permit cycle. This particular value occurred during influences of hurricane Florence, so no actions
were taken. The average flow for the permit cycle was around 0.4 MGD, so one-half of the permitted flow.
There were only 46 individual daily flow values that exceeded the permit limit over the permit cycle. The
application indicates an average daily volume from I/I of 29,000 GPD.
BOD Limits - In addition to the BOD limits in the permit, there is a requirement for 65% BOD monthly average
removal. Looking just at the average influent and average effluent it appears that the facility is achieving over
90% removal. The 65% removal requirement is less than the usual 85% removal for POTWs but is consistent
with federal allowances for equivalent secondary treatment standards for certain treatment systems. These
requirements are found at 40 CFR 133.105 and apply to facilities with waste stabilization ponds like the White
Lake WWTP. This is also discussed in Section 5.1 of the EPA NPDES Permit Writers Guide.
TSS Limits -The effluent limits for TSS of 90 mg/L MA and 135 mg/L WA in the permit are set consistently with
the equivalent secondary treatment standards mentioned above for waste stabilization ponds. 40 CFR
133.103(c) establishes state level standards for TSS. More information is available in Section 5.1 of the EPA
NPDES Permit Writers Guide.
Fecal Coliform Limits - Over the permit cycle the town has had only one effluent limit violation for fecal. This
violation resulted in an enforcement action.
Total Residual Chlorine Limits - There have been no violations of the 17 µg/L limit. All values but one over the
permit cycle were reported at < 10.
The 2017 permit renewal mistakenly included Daily Maximum effluent limitations. These have been changed
back to the required Weekly Average limits in this renewal.
Discussed Sludge Depth Survey condition in the permit with the Fayetteville Regional Office (Call with Mark
Brantley - 4/11/2022. The region wants to maintain this condition in the permit.

FILE HISTORY REVIEW SUMMARY
• July 27, 1992 Authorization to Construct (ATC) construction of a chlorination basin with gaseous chlorine
feed equipment and for the conversion of the existing ultraviolet disinfection basin into a dechlorination
basin with sulfur dioxide feed.
• The Waste Load Allocation (WLA) done in 1995 was based on 0.8 MGD flow and set limits at: BOD - 30; DO
- >5; TSS - 90; Fecal - 200; pH - 6-9 along with monitoring for NH3-N.
• 1995 Permit Renewal - set limits with monthly averages as stated in the 1995 WLA and with weekly
average limits of: BOD - 45; TSS -135; Fecal - 400. The permit also required monitoring for NH3-N, TRC,
Temp, TN and TP. Additionally, U/D monitoring was required for DO, Fecal, Temp and Conductivity. BOD
removal is required at a monthly average removal of 65%.
• 2002 Permit Renewal - removed monitoring for Conductivity and for U/D monitoring for Fecal. This permit
required the development of a Wastewater Management Plan six months after the effective date of the
permit. This plan required a comprehensive facility assessment and optimization of existing facilities due to
poor compliance. The permittee was warned that high total residual chlorine levels must be reduced.
• Classification Letter - August 26, 2004 - This letter notified the permittee of classification as a Grade I
Biological Water Pollution Control Treatment System.
• 2006 Permit Renewal - Permit effective date of February 1, 2007. Treatment components in the permit
were modified. Effluent limits were changed to establish a 17 µg/L daily max limit for TRC beginning in
August of 2008 (eighteen -month compliance schedule). The requirement for a Wastewater Management
Plan was removed and a new special condition was added for a Nutrient Re -Opener
• 2012 Permit Renewal - Treatment plant components were updated again. Added footnote for TRC
compliance at the 50µg/L level. This renewal added a special condition requiring a Solids Depth Survey.
• 2017 Permit Renewal - This permit had an inadvertent change of the headings in the effluent table to
indicate "Daily Maximum" limits but it should have been "Weekly Average." These limits remained correct
in BIMS and were implemented as Weekly Averages. This permit added language requiring electronic
reporting of DMR data and removed instream monitoring for DO and Temperature based on past
compliance with these limits. In addition, consistent with the facility classification as a Grade I Biological
System, monitoring frequencies were adjusted.
PROPOSED PERMIT CHANGES:
• Updated eDMR requirements to be consistent with final EPA rule.
• Added regulatory citations throughout the permit as needed.
• Corrected permit limits to note weekly average rather than daily max limits. This had been mistakenly
changed in the 2017 permit.
• Nutrient Reopener language updated.
• Updated the permit map including correcting the lat/long coordinates in the map legend to match the
location on the map.

Just wanted to touch base quickly on a couple of things for the White Lake renewal. First of all, in my contacts with
them it appears that they are going through some changes with staff there due to retirement. That impacts staff that
have been involved with the wastewater program. The Town Administrator (Sean Martin) may have reached out to you
guys as he is working through getting some contact info updated. Just thought you should be aware since they might
need some technical assistance. Sounded like they were going to coordinate with another community near them to
have a temporary ORC.
In the permit language I also wanted to check with you on a condition that required White Lake to do a solids depth
survey in the last permit. Looks like that has been in the last two permits. Is this a condition that you think should
continue to be in the permit? Let me know what you think.

I have attached a number of documents for you after our discussion yesterday. As we discussed, you have a good bit of
turnover with retirements from the Town and you need to make changes in the permitting system. Below is a brief
explanation of what is attached.
Delegation of Signature Authority — there are two files attached, one a pdf and one a word version. This form is
used for your person with legal authority (mayor in your case) to delegate signature authority for permitting
actions to other staff.
Electronic Signature Agreement — two files — pdf and word. This form is used to grant authority for staff to be
able to submit Discharge Monitoring Reports in our electronic eDMR system.
• ORC Designation form — fillable pdf only. This form will allow you to update your ORC designation.
I am still waiting to get a contact person in our Operator Certification Program for you in case you have questions about
the ORC change process. I have copied here Mark Brantley so you have his email. He is the Assistant Regional
Supervisor in our Fayetteville Regional Office. He is a good contact person for questions you might have as you deal with
the transition of staff currently responsible for your NPDES wastewater permitting activities. You can also reach him in
the office at 910-433-3326.
You can also contact me if you have any questions as we go through the permit renewal process. It is a little easier right
now to try and first contact me through email and then we can set up a call if needed.

I am the permit reviewer for the permit renewal for the Town of White Lake's WWTP — NC0023353. I'm sorry for the
time it has taken to get to this review, but I am starting the process now. There are a few things, outlined below, that I
wanted to touch base with you about.
Additional Information:
In my initial review of the application package I noticed that the application (From 2A page 12) was not signed. Due to
some regulatory requirements at this time we are required to have an actual signed version of the application in our
files. Could you please submit a signed page 12 of the Form 2A application to our offices. You only need to send this
page, not the whole application. You can send that to my attention at the address below. If you need met to send you a
copy of that page of your application for signature let me know and I can send it through email.
Electronic Transmittal of Documents:
In order to provide more convenience, control, and security to our permittees and assist them in processing their
transactions, The Division of Water Resources is currently transitioning towards electronic correspondence. This will
hopefully provide more efficient service to our permittees and other partners and will allow us to more effectively
process and track documents. We are writing to ask you for your approval of the transmittal of documents related to
your permitting and related activities with the Division in an electronic format. Documents will be emailed to the
appropriate contact person(s) in your organization in a PDF format.
Please respond to me through email with verification that transmittal of your documents in an electronic manner is
acceptable to you. If you have any questions, please feel free to contact me.
If you have any questions or want to discuss this or the renewal process, please let me know. At this time it is best to
contact me through this email address. If we needto set up a call to go over anything we can set that up through email.
