Dear Mr. Montebello,
The City of Graham request renewal for Collection System Permit No. WQCS00065 for the
operation of the City of Graham Collection System. Please find enclosed one original and one digital
copy of the permit application, form CSA 08-13, along with the following required items and
attachments in the City's application package.

The Utilities Director has previously been designated as the authorized signing official by the
City Manager as required by 15A NCAC 02T.0106. Currently,the individual in this position is Tonya A.
Mann. An updated Delegation Letter is included due to a change in City Manager.
The City currently has a total of six (6) sewer pump stations receiving flow and in service at this
time. The City of Graham recently installed a new gravity sewer outfall that was extended along Back
Creek from the Cherry Lane Pump Station to Back Creek#1 Pump Station. The new sewer outfall was
connected to the manhole upstream of Back Creek# 1's existing wet well. This manhole connection
diverts sewer flow to the new outfall and away from the Back Creek#1 wet well. Back Creek#1 sewer
pump station was removed from service in February 2022.
In April of 2021, a new Boyd Creek Pump Station was placed in operation.The new station was
constructed directly adjacent to the old pump station at the end of Ingold Dr.The new pump station has
submersible pumps capable of pumping 1,000 gpm during normal operation. An automatic bar screen
was incorporated into the design to capture larger solids and debris before entering the wet well. An
automatically activated diesel backup pump provides redundancy in the event of a power outage. A
water line was also extended to the pump station to make routine maintenance easier on staff.
All of the pump stations currently have permanent generators installed except for Boyd Creek
which has an automatically activated diesel backup pump. The City has recently awarded a contract for
the installation of a permanent generator at the new Boyd Creek Station. A portable generator is
available if needed until the installation of the permanent generator is completed. Additionally,the City
is a member of NC WARN, a statewide coalition to provide mutual aid assistance in the event of an
emergency.

Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16& SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.

C. System-Wide Wastewater Collection System (FORM: CSA 04-16)Application:
❑ Submit the completed and appropriately executed System-wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section 1.3 is a Privately-Owned Public Utility, provide the Certificate of Public
Convenience and Necessity(CPCN)from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

➢ The Authorized signing official listed in Section I.4 should match with that of the Applicant certification page in
accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated
as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T
.0106(b).

CITY OF GRAHAM SEWER SYSTEM
NOTIFICATION POLICY FOR A SANITARY SEWER
SPILL OR OVERFLOW
Any spill of 1000 gallons of wastewater or more; or any spill
of wastewater, regardless of volume, that reaches the
surface waters of the State must be reported to the State.
Depending on the severity of the spill or bypass it may be
necessary to take measures to isolate and/or treat the
discharge. In order to assure the proper response, the
following procedures should be followed:
If ANY spill/bypass is discovered call:

Please give the location of the spill and tell if the wastewater
is flowing into any waterway (creek, drainage ditch, storm
drain, or river).
Upon being notified of a spill/bypass the employee will refer
to the Spill/Bypass Response Policy (Page 2) for further
guidance.

WASTEWATER SPILL/BYPASS
RESPONSE POLICY
Upon notification of a wastewater spill or bypass, personnel
will respond to the site immediately. The initial assessment
must be within 60 minutes of notification. The first responder
to the site will assess the situation and determine if the
spill/bypass is:
(A) More than 1000 gallons;
(B) Reaching any waterway and/or
(C) Potentially causing an environmental or health hazard.
If none of the above [(A), (B) nor (C)] is true (nor has
the potential to become true for this event) then take
the following actions:
1 ) Contact appropriate personnel needed to:
a) Secure the area
b) Correct the problem that is causing the spill
c) Contain the spill
d) Clean the area of the spill (see page 4 for procedure)
2) Document information about the spill on the "Sewage Spill
Response Evaluation" form. (Does not have to be
reported to State since spill is less than 1000 gallons and
is not reaching waterway, but must be documented and
filed).

If only (A) is true then take the following actions:

event) then take the following actions:

d) Clean the area of the spill (see page 4 for procedure)
2) Document information about the spill on the "Sewage Spill
Response Evaluation" form. (Does have to be reported to State
since spill is reaching waterway).

If (C) is true then take the following actions:
1 ) Report the spill immediately to the NCDEQ WSRO at 336-
776-9800. After hours contact the Division of Emergency
Management at 1 800-858-0368 24-hr.
2) Contact appropriate personnel needed to:
a) Secure the area
b) Correct the problem that is causing the spill
c) Contain the spill
d) Clean the area of the spill (see page 4 for procedure)
3) Document information about the spill on the "Sewage Spill
Response Evaluation" form. (Does have to be reported to State
since spill is reaching waterway).
CLEAN UP PROCEDURES
1 . Remove all visible debris
2. Remove all visibly ponded wastewater
3. Disinfect the affected area (NO CHEMICAL ADDITION
TO SURFACE WATERS)
4. Barriers (i.e. colored tape) should be erected to
preclude public access for at least 24 hours.
5. If soil is disturbed in close proximity to stream banks,
the area should be reseeded immediately to prevent
erosion

SEWER LINE SSO - RESPONSE & REPAIR PLAN
1 . Upon being notified of a sewer spill respond to the
scene immediately for an initial assessment.
2. Notify Kenny Hill or Ladd Nall of the situation.
3. Follow the guidelines specified for the type of spill as
outlined on pages 2 and 3 of this policy.
If the spill or overflow is a result of a lift station problem,
call Kenny Hill. If no response, call Ladd Nall or Seth
Hyatt.
If additional help is needed to contain the spill call C-Com
(911 ) and have them dispatch Graham Fire Dept. Haz
Mat Division.

LIFT STATION SSO - RESPONSE & REPAIR PLAN
1 . Upon being notified of a lift station overflow immediately
call the members of the response team.
2. Upon receivingthis call, report immediatelyto the WWTP.

3. Using the response procedures checklist, ready the spill
kit and trailer.
4. When members arrive, hitch the trailer to the truck and go
to the site of the spill (never try to contain the spill alone).
If additional help is needed to contain the spill call C-Corn
(911 ) and have them dispatch Graham Fire Dept. Haz Mat
Division.

LIFT STATION SSO
FOR THE SPILL SCENE
1 . Load bag(s) of dehydrated Lime onto the response vehicle
to use at the spill site.
2. At the spill site, eliminate the source of the spill if possible.
3. Determine how to prevent discharge from getting into any
waterway.
4. Use any available equipment to prevent discharge into the
waters of the state.
5. Treat and clean up the spill using a bag(s) of dehydrated
Lime as needed.
6. If requested by the superintendent on site or state official
— Take upstream and downstream samples (Use the
checklist in the spill kit).

REPORTING POLICY AND PROCEDURES
The City of Graham will report all sewer spills and plant bypasses that
meet the following criteria:
1. Any spill greater than 1000 gallons or,
2. Any spill that reaches the waters of the state.
Reporting procedure for spills and bypasses of the City of
Graham
1 . Highest person in authority at the spill or bypass will give an oral report
to (1) Ladd Nall (2) Cris Routh (3) Tonya Mann or (4) Shelby Smith. The
following information is to be reported:
a) Date, time, location, and source of spill or bypass.
b) Approximate number of gallons.
c) Duration or expected length of time the event lasts.
d) Action taken to contain the spill or clean up.
e) Results if any sample analysis was performed.
2. Personnel named above shall do the following:
a) Contact the Winston-Salem Regional Office (WSRO) and make an oral
report of the information listed above within 24 hours of first becoming
aware of the event.
b) Make sure the state form "Sewage Spill Response Evaluation" is
completed.
c) Within 5 days of the oral report submit the completed form to the WSRO.
3. Additional reporting requirements if spill exceeds 100,000 gallons
or fish kill is observed:
Notify the Division of Emergency Management, they will contact the
Division of Water Quality (DWQ) staff who will respond to us.
