P OST O FFICE B OX 146 R ONDA, N ORTH C AROLINA 28670 Subject: Permit No. WQ0037135 U.S. Biosolids Class B Residuals Program Attachment A Certification Wilkes County Dear Mr. Key: In accordance with your permit major modification request received May 20, 2022, we are forwarding herewith a modified Attachment A certified August 9, 2022, to U.S. Biosolids, Inc. for the subject residuals management program. Modifications to the subject Attachment are as follows: source, with an approved annual dry tonnage of 200 dry tons per year (DT/yr). Increase in the approved annual Street WWTP (Permit No. NC0020761) from 218 to 300 DT/yr. The modified Attachment A shall be effective from the date of certification until September 30, 2024 (i.e., expiration date of the current permit), shall replace Attachment A of Permit No. WQ0037135 issued July 23, 2020, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above-referenced permit. Please replace Attachment A of Permit No. WQ0037135 that was issued July 23, 2020 with the modified Attachment A. Please note that the Permittee shall submit a renewal application no later than April 3, 2024. If any parts, requirements or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
