USGS Quad No.: E 14 SW 7. Size (land available for expansion and upgrading): The area available for expansion and upgrading appeared to be limited. 8. Topography (relationship to flood plain included): The topography contained gently rolling hills sloping toward the current receiving stream. The facilities do not appear to be located in the flood plain. 9. Location of Nearest Dwelling: There are no dwellings located within 500 feet of the treatment plant. Page Two 10. Receiving Stream or Affected Surface Waters: Proposed: Option 1 - Pinch Gut Creek; Option 2 - Clarks Creek a. Classification: C b. River Basin and Subbasin No.: Catawba; 03 08 35 c. Describe receiving stream features and pertinent downstream uses: Both proposed receiving streams contained a good flow on the inspection date. The Town of Maiden's discharge (N00039594) is immediately downstream of both proposed discharges. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. Type of Wastewater: 0o Domestic 100% Industrial a. Volume of Wastewater: 1.0 MGD (Design Capacity) b. Types and quantities of industrial wastewater: Approximately 1.0 MGD of wastewater is produced from the dyeing, bleaching and finishing of fabric (cotton and synthetic). c. Prevalent toxic constituents in wastewater: Prevalent constituents that may be toxic are chlorides, sulfates, copper and chlorine bleach. The facility has failed all chronic toxicity testing since July, 1988. A Special Order by Consent has been drafted and sent to the facility, but has yet to be signed. d. Pretreatment Program (POTWs only): N/A 2. Production Rates (industrial discharges only) in Pounds: The plant is producing approximately 70,000 lbs/day. 3. Description of Industrial Process (for industries only) and Applicable CFR Part and Subpart: The industrial process consists of dyeing and bleaching of cotton and synthetic fabrics. CFR Part 410 Textile Mills; Subpart E: Knit Fabric Finishing 4. Type of Treatment (specify whether proposed or existing): The existing facilities consist of a heat exchanger, a bar screen, dual aeration basins, dual secondary clarifiers, flow equalization pond, post aeration and disinfection facilities. Sludge dewatering equipment is also available. 5. Sludge Handling and Disposal Scheme: Sludge is disposed of by land application with in-house equipment (terra-gator) on Delta Mills property and adjacent pastureland owned by the Jenkins family. The non -discharge permit number is 7120R2. 6. Treatment Plant Classification: Less than 5 points; no rating (include rating sheet). Class III Page Three 7. SIC Code(s): 2253, 2254 Wastewater Code(s): Primary: 02 Secondary: PART III - OTHER PERTINENT INFORMATION 1. Is this facility being constructed with Construction Grants Funds (municipals only)? No 2. Special monitoring requests: No 3. Additional effluent limits requests: No 4. Air Quality and/or Groundwater concerns or hazardous waste utilized at this facility that may impact water quality, air quality or groundwater? No Air quality, goundwater or hazardous waste concerns are expected. 5. Other: N/A PART IV - EVALUATION AND RECOMMENDATIONS The applicant, Delta Mills, Inc., has requested a relocation of its discharge from the present receiving stream (Shady Branch) to one of two proposed locations - Pinch Gut Creek or Clarks Creek. They also are requesting that each proposed discharge location have a waste load allocation prepared for 0.75, 1.0 and 1.25 MGD (request forthcoming). There do not appear to be any problems associated with moving the discharge to either of the proposed locations. However, the facility has failed every chronic toxicity test since July, 198k and Clarks Creek would provide a greater dilution factor. Also, the distance between the proposed Pinch Gut discharge to the Clarks Creek discharge is approximately 200-300 yards and the difference in dilution from the discharge location to the other is quite substantial. Either proposed discharge location would be suitable for discharge, however, this Office would want to review the final waste load allocation before recommending one site over the other. /i' qfq n Signature of Report PrJparer Date /2; /// 2 v Water Qualit e ional Supervisor Date • 1 3 3937 • 2_~_riW J 3934 32'30" • • !!!! The North Carolina Ceriodaphnia chronic effluent bioassay procedure (North Carolina Chronic Bioassay Procedure - Revised *September 1989) or subsequent versions. The effluent concentration at which there may be no observable inhibition of reproduction or significant mortality is /d % (defined as treatment two in the North Carolina procedure document). The permit holder shall perform quarter(v monitoring using this procedure to establish compliance wtnth the permit condition. The. first test will be performed after thirty days from issuance of this permit during the months of Mae AN sew M . Effluent sampling for thus testing shall be performed at the NPDES permitted final effluent discharge below processes. • All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter code Additionally, DEM Form AT -I (original) is to be sent to the following address: Attention: Environmental Sciences Branch North Carolina Division of Environmental Management P.O. Box 27687 Raleigh, N.C. 27611 Test data shall be complete and accurate and include all supporting chemical/physical measurements performed in'association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream. Should any single quarterly monitoring indicate a failure to meet specified limits, then monthly monitoring will begin immediately until such time that a single test is passed. Upon passing, this monthly test requirement will revert to quarterly in the months specified above. Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Environmental Management indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include alternate monitoring requirements or limits. NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism survival and appropriate environmental controls, shall constitute an invalid test and will require immediate retesting(within 30 days of initial monitoring event). Failure to submit suitable test results will constitute noncompliance with monitoring requirements. 7Q10g cfs Permtted Flow O.
