The only change in this permit regards ownership. All other terms and conditions in the original permit remain unchanged and in full effect. This permit modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency dated December 6, 1983. This permit does not affect the legal requirement to obtain other permits which may be required by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Mangement Act, or any other Federal or Local government permit that may be required. Box 29535, Raleigh, North Carolina 27626-0535 Telephone 919-733-7015 FAX 919-733-0719 An Equal Opportunity Affirmative Action Employer 50% recycled/ 10% post -consumer paper Permit No. NC0065412 STATE OF NORTH CAROLINA DEPARTMENT OF ENVIRONMENT, HEALTH, AND NATURAL RESOURCES DIVISION OF ENVIRONMENTAL MANAGEMENT PERMIT TO DISCHARGE WASTEWATER UNDER THE NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the Federal Water Pollution Control Act, as amended, REA Enterprises, LLC is hereby authorized to discharge wastewater from a facility located at NC Highway 150 sothwest of Williamsburg Greensboro Rockingham County to receiving waters designated as Little Troublesome Creek in the Cape Fear River Basin in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III, and IV hereof. The permit shall become effective February 14, 1997 This permit and the authorization to discharge shall expire at midnight on January 31, 2001. Signed this day February 14, 1997 A. Continue to operate an existing wastewater treatment facility consiting of an influent pumps, bar screen, two clarifiers, digestion tank, chlorinator, and flow measurement, located at REA Enterprises, LLC, on NC Highway 150, southwest of Williamsburg, Rockingham County (See Part III of this Permit), and 2. Discharge from said treatment works at the location specified on the attached map into Little Troublesom Creek River which is classified Class C-NSW waters in the Cape Fear River Basin.
