Dear Mr. Misuraca:
The Division received your application for a wastewater discharge permit on February 13, 2006. Division
personnel have reviewed and approved your application. Accordingly, we are forwarding the attached NPDES
discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated May 2004, and as subsequently amended.
A Total Residual Chlorine (TRC) limit has been added to this permit. Because you may need time to make
modifications to your plant in order to comply with this new condition, the TRC limit will not take effect until
December 1, 2007.
Be aware that Little Troublesome Creek is listed as an impaired stream on North Carolina's 303(d) list.
This means that the stream does not meet all water quality standards. The Division is working toward
the improvement of water quality in Little Troublesome Creek. If the Division's current initiatives do not
result in improved water quality, removal of discharges to Little Troublesome Creek may be necessary
for the health of the stream. Although the Division believes urban runoff is the main cause of
degradation to Little Troublesome Creek, we will continue to monitor compliance at your facility.
Please note that this facility discharges to a nutrient -sensitive water (NSW). In 1997 the Clean Water
Responsibility Act (House Bill 515) was enacted, with the intent to strengthen the control of nutrients from all
discharges to NSW waters. Facilities permitted to discharge greater than 500,000 gallons per day (and any
new or expanding facilities) will be subject to nutrient limitations for total nitrogen and total phosphorus
according to House Bill 515 requirements. Since modifications to treatment systems are expensive and time
consuming, we strongly recommend that you begin planning for this possibility now if a future expansion is
anticipated.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of
the North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, North Carolina 27699-6714. Unless such demand is made, this decision shall be final and
binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area
Management Act or any other Federal or Local governmental permit that may be required. N°n` Carolina

revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under

WASTEWATER PERMIT
On the basis of thorough staff review and application of NC General
Statute 143.21, Public; law•,"92-500 and other. lawful standards and
regulasipns",the North Carolirla Environmental Management Commission
proposes to issue a National'Pollut8nt Discharge Elimination System`-
(NPDES) wastewater discharge permit to the person(s) listed below
effective 45 days from the publish date of this notice.
Written comments regarding the proposed permit will be accepted until
30 days after the publish date of this notice. All comments received
prior to that date are considered in the final determinations regarding
the proposed permit. The Director of the NC Division of Water Quality
may decide to hold a public meeting for the proposed permit should the
Division receive a significant degree of public interest.
Copies of the draft permit and other supporting information on file used
to determine conditions present in the draft permit are available upon
request and payment of the costs of reproduction. Mail comments and/or
requests for information to the NC Division of Water Quality at the above
address or call the Point Source Branch at (919) 733-5083, extension
520 or 363. Please include the NPDES permit number (attached) in and
communication. Interested persons may also visit the Division of Water
Quality at 512 N. Salisbury Street, Raleigh, NC 27604-1148 between the

for Pleasant Ridge WWTP in Rockingham County. This permitted facility
currently discharges 23, 500 gallons per day treated wastewater into Little,
Troublesome Creek in the Cape Fear River Basin. Currently, BOD, Total
Suspended Solids, ammonia nitrogen, Fecal Coliform, Total Phosphorous,
and Total Residual Chlorine are water quality limited. This discharge may
affect future allocations in this portion of the Cape Fear River Basin.

I checked our files. Pleasant Ridge, Permit NC0065412 in Rockingham County was
classified as Biological Wastewater Grade February 23, 2001. I do not know how
Grade4 got in BIMS. I will correct this.

and I noticed that BIMS has it classified as WW-4. However, in the current permit,
which expires at the end of April, 2006, they are permitted to discharge 0.0235 MGD
of wastewater and the monitoring frequency for the permitted parameters is for a
class 2 facility. Could you please let me know if the listing in BIMS is correct and
if so, why is the facility rated as class 4.

The addresses in BIMS are in Ohio, but the last permit went to Michael Power in Virginia. I sent
him a permit renewal notice on September 20th and got no response.
The renewal application is now overdue [it was due in by 11/1/2005], so an NOV should be sent.
But to whom, and where?
Any help you can give would be greatly appreciated.
