This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated December 6,1983. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding. Please take notice this permit is not transferable. Part II, E.4. addresses the requirements to be followed in case of change in ownership or control of this discharge. Dispo� Alternatives The Permittee shall cbn-t.�ually evaluate all wastewater disposal alternatives and pursue the most environmentally sound alte ve of the reasonably cost effective alternatives. If the facility is in substantial non-compliance with terms and conditions of the NPDES permit or governing rules; regulations, or laws, the permittee shal11submit a report in such form and detail as required by the Division evaluating these alternatives and a plan of action within sixty (60) days of notification by the Division. �` Permit Summary NC0028711 This is a permit renewal for an existing class II facility. Compliance was achieved for the :most part with parameters within the past twelve months. Chlorine levels within the past twelve months may be of some concern. Had a maximum level of 1466.000 ug/1 and an average of 272.214 ug/1. The region needs to comment on the need for a residual chlorine limit and/or dechlorination requirement for this renewal. Downstream values for DO looked good. DO values were in the range of 5 to 6 during the latter months of the summer, but never below 5.0 mg/1.
