Division personnel have reviewed and approved your application for renewal and
modification of the subject permit. Accordingly, we are forwarding the attached final NPDES
discharge permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-
6714). Unless such demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act, or any other
Federal or Local governmental permits which may be required.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and

As of this permit issuance, any previously issued permit bearing this number is no longer effective.
Therefore, the exclusive authority to operate and discharge from this facility arises under the permit
conditions, requirements, terms, and provisions included herein. .
North Carolina State University is hereby authorized to:
1. Continue to operate an existing 0.0084 MGD wastewater treatment facility with the
following components:

2. After receiving an Authorization to Construct from the Division, expand the facility's
treatment capacity to 0.0131 MGD to operate a wastewater treatment facility with the
following components:
• Septic tank
• Dosing tank
■ E-Z Treat System
■ Ultraviolet disinfection system
3. Discharge from said treatment works at the location specified on the attached map into
Carroll Creek, dassified C waters in the Roanoke River Basin.

Please find enclosed three copies of NPDES Application - Form D, Engineering Alternatives
Analysis, and the 7Q10 low flow characteristics for Carroll Creek. With this application we are
requesting to expand the current permit flow allocation from 0.0084 MGD to 0.0131 MGD. The
camp is going to replace the existing recirculating sand filter with an E-Z Treat recirculating
media system.
During the peak summer months, the camp is experiencing flows at and occasionally exceeding
their permitted limit. We believe part of this is due to rainwater entering the system through the
open sand filter. The proposed E-Z Treat system will prevent rainwater from entering the system
with the covered pods. The 0.0131 MGD is requested so that the existing septic tank can still be
used, and to cover the design flow. Based off the regulations the design flow is approximately
12,360 gallons per day during the peak summer camp months. We feel that this submittal is
sufficient for you to complete your review. If you have any questions or require additional

9. Describe the treatment system
List all installed components, including capacity, provide design removal for BOD, TSS, nitrogen and
phosphorus. If the space provided is not sufficient, attach the description of the treatment system in a
separate sheet of paper.
The original system, designed in 1971, consisted of a septic tank, a dosing tank with two
alternationg siphons, two sand filter beds and chlorination facility. The septic tank has
a capacity of 13,128 galons and the dosing tank a capacity of 2,918 gallons. The system
was modified in 1996 and 1997. The sand filter was revised, new discharge and
collection lines were installed along with new layers of sand and gravel, and the pvc
piping was placed in the center of a 10-inch layer of gravel.
The effluent from the filter bed is collected through a gravity system and it flows to the
recirculation and discharge wet well. It is then pumped again to the sand filter, where it
is sprayed on top of the filter bed where it flows through the sand filter. It discharges
once more to the wet well, where the discharge pump sends it through the UV
disinfection system, and then on to Carroll Creek by gravity.

A proposed upgrade to the system is expected upon a thorough analysis and the result of
the flow allocation request. We are requesting that the flow be increased to 13,100
gallons per day for growth since the plant is being upgraded. This is due to the size of
the existing septic tank, which is the peak flow for the camp would be calculated as
follows.
200 campers * 60 gallons per camper = 12,000
3 bedroom residence * 120 gallons per bedroom = 360
Additional Flow for Future Expansion = 740
Total flow = 12,000 + 360 + 740 = 13,100 gallons per day.

General Project Information and Background
Project Description
Betsy -Jeff Penn 4-H Educational Center is located in Rockingham County, North Carolina. It is
a camp operated by North Carolina State University through the Department of 4-H Youth
Development. The camp currently has a NPDES permit (NC0078271) for 8,400 gallons per day.
The current wastewater treatment system consists of a pump tank, septic tank, recirculating sand
filter, ultraviolet disinfection unit, and a gravity discharge to Carroll Creek.
As the existing sand filter ages, it becomes less effective at treating the wastewater. To avoid
permit compliance problems, the camp wants to replace their current filter with an E-Z Treat
Treatment Center by E-Z Set Company. The E-Z Treat Treatment Center is a recirculating
media filter that uses polystyrene beads as the biological growth media. The new system needs
to be designed to meet the camps current needs as well as any possible future expansions We
are requesting that the Division of Water Quality assist us in determining the possibility of
increasing the allowable discharge per day into Carroll Creek. Since the future expansion needs
are not known at this time, the camp would like to request a flow allowance of 13,100 gallons
per day. This is due to the size of the existing septic tank which is 13,100 gallons. The camp
wants to retain the existing tank in their new system.

Population Projections and Flows
The camp's population varies widely throughout the year with the different camp schedules.
Summer camp runs for eight weeks throughout the summer months each year, with
approximately 200 campers each week staying at the camp, for a total of 1,600 per year. During
the summer they also run day camps which total approximately 240 campers per year.
Throughout the year, user groups are held which totaled 5,741 people in 2005. The camp
director and his family (wife, and two kids) live at the camp full time and are also hooked into
the camps wastewater system.
The population figures above are representative of what the camp experiences every year. The
camp wants to install a wastewater treatment and disposal system that will meet their current
needs as well as any future expansion. Currently the camp experiences flows close to and
occasionally exceeding the permitted flow of 0.0084 MGD. On July 5, 2006 the discharge flow
was 12,700 gallons, but July 10 was only 6,600 gallons. The 12,700 gallons may have been due
to high rainfall with the open sand filter. Once the new treatment system is in place, rainwater
will not be able to enter the system.
To help reduce water consumption in the future, the camp is planning to replace old fixtures
(sinks, toilets, etc.) with Iow flow models when they are replaced. Old fixtures are only replaced
when they are broken, so this process will be lengthy but eventually switch the camp over to low
flow fixtures. In the future when the camp replaces old washing machines with new ones, water
conservation will be an added benefit due to the design of the newer machines.
Technologically Feasible Options
Three technologically feasible options exist for wastewater treatment and disposal at Betsy -Jeff
Penn 4-H Educational Center. Option 1 would be to connect to a municipal sewer system. The
camp is claimed by the Town of Wentworth and their nearest sewer line is approximately 6 miles
away. The City of Reidsville has a pump station approximately 3 miles away from the camp, but
would only run lines to the camp if they could annex them. Annexation is very unlikely, because
the Town of Wentworth incorporated a large buffer zone to protect it from Reidsville expansion.
This option would tum into a political fight that is not practical because the camp is not a tax
revenue producing business.
Option 2 would be on -site wastewater treatment and disposal. We believe this to be a bad idea,
because the camp previously had a septic system that failed. That original failed system is what
led to this discharge permit. The third option is to expand the current NPDES permit, to meet the
camp's future needs.
Economic Feasibility of Options
Option 1 is not economically feasible, nor practical. Running 6 miles of sewer line is out of the
5 mile radius requested by the analysis. A non-profit camp with state funding cannot afford, nor
should it be expected to run 6 miles of sewer. If the camp is not allowed to expand its current
discharge flow, it will not seek other options for wastewater disposal. A copy of the 7Q10 flow
report is included with the analysis.

In response to your inquiry about the low -flow characteristics (7Q10) for Carroll Creek below the dam (Lake Hazel)
near University Estates in Rockingham County, the following information is provided:
A check of the low -flow files here in the USGS office indicates the location identified on your fax location map is one
where low -flow estimates were previously estimated in February 1990 (station id 0207429247, drainage area 3.8
sqmi). No USGS discharge records for your point of interest or other locations on Carroll Creek are known to exist.
Low -flow characteristics for ungaged locations are estimated by considering the range of low -flow yields (expressed
as flow per square mile drainage area, or cfsm) at nearby sites where sufficient discharge records are available for
low -flow analyses.
In the 1990 estimate, the 7Q10 low -flow yield was determined to be 0.091cfsm, based on two USGS partial -record
sites on Wolf Island Creek where Carroll Creek drains. Applying the drainage area of 3.8 sqmi, the resulting 7Q10
discharge is estimated at about 0.35 cfs. It needs to be noted the 7Q10 low -flow estimate being provided does not
account for the presence of Lake Hazel upstream of the site. No information is known about the hydraulic
characteristics for this dam and its particular effects, if any, on downstream flows in Carroll Creek.
The most recent (1996) low -flow characteristics published for streams in Rockingham County is included in a low -flow
report for the Roanoke River basin in North Carolina:

Based on table 7 in the above low -flow report, the 7Q10 yield ranges from 0.05 to 0.08 cfsm about at several nearby
USGS partial -record sites on Jacobs Creek and Rockhouse Creek (west of Carroll Creek) and Wolf island Creek
(east). The higher value is based on the nearby site at Wolf island Creek at Reidsville (station id 02074282, drainage
area 3.71 sqmi). Both these values are lower than the yield of 0.091 cfsm used in the 1990 estimate, which obviously
raises the question as to what low -flow yield is appropriate for Carroll Creek.
Because there are no discharge records on Carroll Creek, it appears the most appropriate means of estimating the
7Q10 discharge would be to use the nearby range 0.05 to 0.08 cfsm. Doing so suggests that some variation appears
to exist in flow characteristics within this area. Further, because Carroll Creek drains into Wolf Island Creek (where
the 7Q10 yield is 0.08 cfsm), leaning towards use of the higher value in the yield range could be justified. And use of
this yield value would be close to that used in the 1990 estimate.

Gil- here's the compliance feedback from WSRO on the facilities that we highlighted with
potential compliance problems in the Roanoke Basin. Of the 20 listed facilities, WSRO
indicates that 2 have connected, and 2 should have a Special Condition placed in the
draft permit renewal requesting a Wastewater Management Plan to address compliance
problems (see below):

alert the assigned permit writers of the additional requirements. For the remaining
Roanoke Basin, I've already received feedback from WARO, and I'm still awaiting RRO
feedback (due to us by Oct. 6).

The Roanoke Basin (subbasins 030201-030210) is next in line for NPDES wastewater
permit renewals, with 77 permit renewals falling within three regions: WSRO (n=47),
RRO (n=18), and WARO (n=12). The Roanoke permits begin to expire 1/31/2007, which
means that NPDES staff will start sending drafts to public notice in December06.
NPDES staff attempts to incorporate compliance into permit renewals by reviewing the
compliance record to identify problem facilities, and discussing with the applicable
Regions whether the permit renewal should contain any special conditions to address
continuing compliance problems.
Similar to the Cape Fear renewals, I reviewed a BIMS download of enforcement cases
during 2001-05, and used a subjective compliance threshold of 3+ enforcement cases
over the last 2 years, or 5+ enforcement cases over the past 5 years, to identify
potential problem facilities. This threshold resulted in 30 facilities, spread between
WSRO (n=20), RRO (n=7), and WARO (n=3). These facilities are listed by Region in the
attached spreadsheet. The next step is for NPDES/Region to review the facility lists
and determine what permitting conditions, if any, would be appropriate for each listed
facility. For some facilities, the cases may be older and the problem may already be
resolved, or is currently being addressed through SOC, etc, in which case no
permitting action is necessary. The most common permiting action for a problem
facility has been to add a Special Condition requiring preparation of a Wastewater
Management Plan that focusus on specific problems and how the facility plans to
resolve them.
I'll leave it to the Regions to determine how they want to proceed. Regions can
either review their listings and enter comments on the spreadsheet and send back, or
NPDES can call/visit the Region to discuss each listed facility. Both ways have
worked in the past. Either way, I'd like to complete this task BY SEPTEMBER 30,
before NPDES staff begins the permit renewals. Any permitting conditions deemed
appropriate by Region/NPDES will then be placed in the permit renewal files, and
ultimately incorporated into the draft permit. If Regions have any questions, feel
