Page# 1 31 NPDES yr/mo/day N C 0047384 111 121 22/08/02 117 Inspection Type 18H (Cont.) 1 Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On August 2, 2022, Alex Lowe of the DWR WSRO conducted a routine pretreatment compliance inspection at the City of Greensboro's T.Z. Osborne WWTP. Onsite representatives present were Amy Varinoski (Pretreatment Coordinator), Martie Groome (Lab & Industrial Waste Supervisor), Bill Burdick (Compliance Specialist), and Alicia Goots (Lab Coordinator). A review of the pretreatment program elements indicated that all program elements have been updated an submitted to the State; both the headworks analysis (submitted February 15, 2022) and the enforcement response plan (submitted March 1, 2022) are still awaiting approval. Compliance files were reviewed for three SIUs: Qualicaps, Lanxess, and Ecolab. Ms. Varinoski noted that Lanxess was found to be the source of the 1,4-dioxane exceedence reported by the WWTP in April of 2022, and will be responsible for paying the fine associated with the SOC once it is assessed by the State. Ms. Varinoski also explained that Ecolab was held responsible for a large upset at T.Z. Osborne after it was reported that concentrated anti -bacterial material was accidentally discharged from their facility. The root cause was determined to be a drainage valve on the containment area that was left open during the delivery of raw materials, coupled with a small leak in the hose used to offload the chemical from truck to storage tank. Ms. Varinoski noted that the SIU has been fully cooperative in remediating the issue, and will take full financial responsibility to replace the bugs killed off in the discharge and cover the cost of civil penalties from the State. Lastly, Ms. Groome shared that she will be retiring from her position at the end of August 2022. She will return to the city sometime in October 2022 on a part-time basis as a special projects manager working to maintain progress on the 1,4-dioxane SOC. No instances of non-compliance were uncovered during this inspection, and as such this program is considered stisfactory. Page# 2
