Division of Water Resources' Central and Regional staff has reviewed the application package
received May 5, 2022. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on September 1, 2022.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A and B, or failure to provide the additional information on or before the above requested date
may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Reclaimed Water System —Renewal Form (FORM: RWS-R 02-21):
In accordance with 15A NCAC 02T .0106(b)(4), permit applications shall be signed by an
executive officer, elected official in the highest level of elected office, or an authorized employee
in the case of a public entity. This application was signed by Benjamin Lassiter, the Operations
Manager for the Town, but he is not listed as an authorized employee in our system. This
application shall be resubmitted so that the Applicant's Certification is done by a signature
authority (in this case, either a mayor or a town manager). Alternatively, a signature authority
delegation letter can be submitted in accordance with 15A NCAC 02T .0106(c).
B. Property Ownership Documentation:
This project includes reclaimed water distribution lines connecting the treatment plant on parcel
3792-58-8548 to the irrigation field on parcel 3792-69-5506, both owned by the Town of
Macclesfield. These parcels are not contiguous, meaning the reclaimed water distribution lines in
the permit description would need to cross property lines onto land not owned by the Permittee,
though it is not clear exactly where these lines are located. In accordance with 15A NCAC 02T
.0116(c), all deeds, easements, and encroachment agreements necessary for installation, operation
and maintenance of the system shall be provided. Please provide any necessary utility easements
needed for the operations and maintenance of these facilities on land not owned by the Permittee.
