Attach a vicinity map clearly showing the location of the property with respect to local
landmarks such as towns, rivers, and roads. Also provide a detailed site plan shaving property
boundaries and development plans in relaHan to surrounding properties. 'Both the vicinity map
and site plan must include a scale and narih arrow. The specific footprints of all buildings,

Vi. Proposed Impacts to Waters of the United States/Waters of the State
it is the applicant's (ar agenk`s) responsibility to detern~ine, delineate and map all impacts to
v~retlands, open water, and stream channels associated with the project. Each impact must be
listed separately in the tables below (e.g., culvert installation should be listed separately from
riprap dissipater pads). Be sure to indicate if an impact is temporary. All proposed impacts,
permanent and temporary, must be listed, and must be labeled and clearly identifiable on an
accompanying site plan. A11 wetlands anal waters, and all streams (intermittent and perennial)
should be shown on a delineation map, whether or not impacts are proposed to these systems.
Wetland and stream evaluation and delineation forms should be included as appropriate.

information related to site constraints such as iopagraphy, building ordinances, accessibility, and

USACE - Tn accordance with the Final Notice of Issuance and Modification of Nationwide
Permits, published in the Federa] Regisker an January 1S, 2002, mitigation will be required when
necessary to ensure that adverse effects to the aquatic environment are minimal. Factors
including size and type of proposed impact and function anal relative value of the impacted
aquatic resource will be considered in determining acceptability of appropriate and practicable
mitigatian as proposed. Examples of mitigation that may be appropriate and practicable inchide,
but are not limited to: reducing the size of the project; establishing and maintaining wetland
and/or upland vegetated buffers to protect open waters such as sgreams; and replacing losses of
aquatic resource functions and values by creating, restoring, enhancing, or preserving similar
functions and values, preferable in the same rrvatershed.
If mitigation is required far this project, a copy of the mitigation plan must be attached in order
for USACE ar DWQ to consider the application complete for processing. Any application
lacking a required mitigatian plan or NCEEP concurrence shall be placed on hold as incomplete.
An applicant may also choose to review the current guidelines for stream restoration in DWQ's
Draft Technical Guide for Stream Work in North Carolina (see DWQ website for most current
version.).
Provide a brief description of the proposed mitigation plan. The description should provide
as much information as possible, including, but not limited to: site location {attach directions
and/or map, if offsite), affected stream and river basin, type and amount (acreage/linear feet)
of mitigatian proposed (restoration, enhancement, creation, or preservation), a plan view,
preservation mechanism (e.g., deed restrictions, conservation easement, etc.), and a
description of the current site conditions and proposed method of construction. Please attach
a separate sheet if more space is needed.

9.0) Describe the overall project in detail, including the type of equipment to be used:
Development of asingle-family subdivision consisting of 41 Lots, 3,558 LF of paved
roads designed to NCD~T standards with roadside ditching, drainage swales and
public water system. Water service will be provided by Onslow Water and Sewer
Authority (ONWASA). Sewage disposal will be by individual septic systems as
approved by the Onslow County Environmental Health Department. The equipment
planned to be used will be the normal construction equipment (i.e. dump truck, motor
grader, dozer, and backhoe).

Two impacts to wetlands will be required to develop this site into single family residential
lots. These proposed impacts are:
(W9} 130 square feet (0.003 ac) at Road Station 5*54 for the installation of a culvert to
construct the main road in the development,
{W2) 4325 square feet (Q.099 ac) at Road Station 11 *60 for the installation of culverts to
construct the main road in the development,
All impacts are permanent.

Vll. Impact Justification {Avoidance and Minimization)
Avoidance measures;
The access point to the public road system far this project site is the Burgaw Highway
{NC 53). The site is bounded to the east by small parcels with wetlands that are
individually owned, to the north by lots in an adjacent subdivision and Blue Creek, 'to
west by a large parcel of undeveloped land with small trees and open fields and to the
south by Burgaw Highway (NG 53). One of the wetkand fingers in this area extends
across the entire width of the propErty from east to west. In order to get to the northern
park of the parcel, the wetlands must be crossed. There are no other public roads
adjacent to this site other than Burgaw Highway (NC 53), thus providing access to this
development will require the crossing of these wetlands.
Alternate site plans were considered, but since they all require access from Burgaw
Highway, they also resulted in filling the wetlands. The other site plans do not reduce
the impact to wetlands and provide a less desirable road alignment and lot configuration
and therefore were rejected.
Constructing a bridge over the wetlands at Engineer's Stataon 11+60 would completely
avoid impacting them, but this would require a bridge span of at least 65 feet. This
would increase the maintenance costs for the roadway and would be cost prohibitive to
the owner. It was therefore was rejected.
It is not possible to avoid all impacts to wetlands and provide access to the proposed
lots.
Minimization measures:
Alternate road design standards were explored, but since these subdivision roads will be
taken over by the North Carolina Department of Transportation Standards they must be
constructed to their minimum standards. These streets will be constructed with a 20-foot
roadway surface, a 6-foot shoulder, and 4:1 side slopes. l-owering the roadway
standards below this would reduce the safety of the road for the residents and visitors to
the proposed single-family residences and would make them ineligible to be included in
the NCDOT Secondary Road System. Therefore reducing the roadway standards is not
a feasible alternative.
Headwalls will be constructed at both the inlet and outlet of the double 48~inch culverts
at Engineer's Station 11+Gfl. This will eliminate the need for flared end sections that
would extend 8.25 feet further into the wetlands, thereby minimizing the impact to the
wetlands.
There are no other minimization measures known.
