On July 2, 2014, the Division of Water Resources (Division) received your application requesting
a 401 Water Quality from the Division for the subject project. The Division has determined that
your application is incomplete and cannot be processed. The application is on- hold ;until all of
the following information is received:
1. The application was not signed. Please provide four (4) signed and dated copies of page
10 of 10 of the application. If the application is not signed by the owner/ applicant,
please provide an agent authorization letter delegating.signature authority. (A sample
agent authorization_ letter can be found online:

2. There was no Nationwide Permit,(NWP) Number or,General Permit (GP) Number listed
on the application. Please contact Eric Alsmeyer ( eric.c.alsmeyer @usace.arm,y.mil ; 919-
554 -4884 x23) with the U.S. Army Corps of Engineers to determine what permit number
'this activity will be covered under. Please provide four (4) updated copies of page 1 of
10 that includes °the NWP or GPrnumber in Part A. 1b.
3. In order for DWR to process the application, a fee of $240 is required. Please remit a
check-for $240 for the correct application fee and reference DWR # 14 -0681 on your
payment and send to the below address.

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested
information for the proper consideration of the application. If all of the requested information
is not received' in writing within 30 calendar days of - receipt of this letter, the Division will be

necessitate reapplication to the Division for approval, including a complete application package
and the appropriate fee.
