In accordance with your application to discharge remediated groundwater (acknowledged received March 4, 2022), the Division of Water Resources (DWR or the Division) hereby forwards this Certificate of Coverage (COC) to discharge under the subject NPDES general permit. We issue this COC pursuant to the requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection Agency (USEPA) dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A copy of the Certificate of Coverage for your treatment facility • A copy of General Wastewater Discharge Permit NCG510000 • A copy of a Technical Bulletin for General Wastewater Discharge Permit NCG510000 • Engineer's Certification, to be signed and returned to DWR upon project completion The Division understands that you have authorized engineering consultant, Hart & Hickman, P.C. (H&H) of Charlotte, to manage your groundwater remediation (GW-REM) and apply for this COC. Information supporting this application includes site background, site assessment analyses, details of dual phase extraction (DPE) system information, and an analysis of alternatives to direct discharge. The permit application package included information provided as: 1. Figure 1 — Site Location Map 2. Figure 2 Site Map 2923 3. Figure 3 — NPDES Discharge Locations Map 4. Attachment A — Designation of H&H as Authorized Representative for the City of Charlotte, Aviation Department (i. e., the permit applicant) 5. Attachment D — Analytical Data for Representative Sample of Water to be Treated North Carolina Department of Environmental Quality I Division of Water Resources 512 North Salisbury Street 1617 Mail Service Center I Raleigh, North Carolina 27699-1617 -- 919.707.9000 DocuSign Envelope ID: 45050B2F-F023-4BDA-A98E-A23512125042 DWR acknowledges your consultant's site-remediation Free Product Corrective Action Plan (CAP), approved by the NCDEQ/Division of Waste Management (DWM) on December 1, 2021, pending further approval by NPDES. The Division notes that you assert, based on site assessment chemical analyses, that the source of site contamination is jet fuel type A (Jet -A), and further that fuel was likely lost to the environment via underground fueling lines [currently out of service] associated with the Fuel Farm above -ground storage tanks (ASTs). The quantity of fuel released is unknown but is estimated to be greater than 100,000 gallons. We note no history of vehicle -repair facilities in the area suggesting that parameters of concern (POCs) would not likely include chlorinated solvents. However, the chlorinated solvent Trichloroethylene (TCE) was detected above its practical quantitation level (PQL= 5.0 µg/L) in monitoring well MW-8. Analyses at this location indicate 6.27 µg/L TCE (see permit application page 11 of 42). P3ease note that if future analyses detect chlorinated solvents in the effluent exceeding surface water quality standards, you must notify the NPDES Unit immediately (attention Joe R. Corporon, P.G.) and apply for revised limits and monitoring conditions. Based on the above, the General Permit's effluent monitoring Table A. (6.) applies to your facility as having been impacted by Jet-4 fuel and other contaminants s as a combined source of hydrocarbons (see attached). It follows that Monitoring Tables A. (1.) through A. (5.) do not apply. Future reporting under this COC shall compare all analytical results to Surface Water -Quality Standards appropriate to each specific analyte and to a receiving -stream classified C. Please note that all chemical analyses must apply laboratory minimum detection levels (MDLs) sufficiently sensitive to embrace each analyte's given standard. At no time shall treatment -system operators allow contaminated (untreated) water to discharge to surface waters of the State. At no time shall treated water discharge such that sediment is transported and/or erosion occurs. The Measurement Frequency "Monthly Flow" is defined as total treated effluent not -to -exceed one million gallons (1.0 MG) per month. This assumes that the system utilizes a totalizing flow meter (or equivalent method) to determine and record total effluent flow, and the Permittee shall report this value on quarterly Discharge Monitoring Reports (DMRs) and on electronic Discharge Monitoring Reports (eDMRs), as required by the Division. Periods of no discharge shall be reported as "No flow" or "No discharge," as appropriate for any required Quarterly DMR or eDMR period. In accordance with your application to discharge and site -assessment Corrective Action Plan (CAP), the Division understands that you propose to construct and operate dual phase extraction (DPE) systems #1 and #2 to remediate groundwater consisting of [but not limited to]: • well array for groundwater extraction [-18 wells (variable)] • DPE w/ demister, blower, and filter • moisture separator w/ pump and totalizing flow meter • flow equalizing tank (1,500 gal; double wall) w/ level switch and skimmer to • product holding tank w/ level switches [high -high; low/low] • EQT transfer pump to oil/water separator • water holding tank (225 gal) • WHT Transfer pump / w totalizing flow meter • bag filter [25 gpm min/ 75 psi min] • three (3) each phased zeolite units (in series) • appurtenant piping to outfall Page 2 of 6 DocuSign Envelope ID: 45050B2F-F023-4BDA-A98E-A23512125042 The above treatment works will be installed at the Airport Drive Release Site, 5414 Airport Drive, Charlotte and operated in accord with the discharge application, as described in text, system schematics and Site Plans attached thereto. Treated groundwater shall discharge to a local stormdrain(s) approved by City of Charlotte Aviation Department under zero -flow receiving -stream conditions (7Q10 and 30Q2 = 0.0 cfs). Effluent ultimately discharges to an unnamed tributary (UT) to Taggart Creek [Stream Segment 11-137-4], a waterbody currently classified C within hydrological unit 03-04-38 [HUC 030501030103] of the Catawba River Basin. If treatment facilities fail to perform satisfactorily, to include creating nuisance conditions, the Permittee shall take immediate corrective action, including those actions as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. Failure to abide by the requirements contained in this ATC may subject the Permittee to an enforcement action by the Division of Water Resources in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. Please notify Corey Basinger of the Division's Mooresville Regional Office (MRO) by telephone 919-704-663- 1699 at least three (3) working days in advance of operating the treatment -system so that he may determine the need for final inspection requirements. Contact MRO during the normal office hours from 8:00 a.m. until 5:00 p.m. Monday through Friday, excluding State Holidays. NPDES Permit Contact. If any parts, measurement frequencies or sampling requirements contained in the general permit are unacceptable, you have the right to request an individual permit by submitting an individual NPDES permit application. Unless such demand is made, the certificate of coverage shall be final and binding. Please note that this Certificate of Coverage (COC) is not transferable except after notice to the Division of Water Resources. The Division may modify, revoke, or reissue this COC as changing conditions or new information may warrant. This permit does not affect your legal obligation to obtain other permits which may be required by the Division of Water Resources, the Division of Land Resources, the Coastal Area Management Act, or any other federal or local government. This certificate of coverage shall become effective August 1, 2022 This Certificate of Coverage shall remain in effect for the duration of the General Permit.
