The final plane and specifications for the subject prof t hav been reviel,.
and found to be eatiafactory. Authoriz-.tion is hereby granted for the construction
of a 0.020 MGD extended aeration wastewater treatment facility consisting of aera-
tion basin, final clarifier with sludge return, effluent chlorination, f ow measur-
ing device, and cascade outfall to serve the Adam's Apple Homeowners Association;
This is a Class I Wastewater Treatment Plant which requires the operator in
responsible charge to hold a valid Grade I Certificate.
This Authorisation to Construct shall be subject to revocation unless the
wastewater treatment facilities are constructed in accordance with the conditions
and limitations specified in Permit No. NC004235$.
Also, enclosed is a copy of WPC Form #50 "Cost of Wastewater Treatment Works."
This form is to be completed and r turned to this office within thirty (30) days
after the. project is completed.
One (1) set of approved plans and specifications is being forwarded to you.
