1) Unless specifically altered herein, this minor modification authorizes the construction of the 40' by
40' building, 8' by 16' deck, 8' by 24' deck, and the on -site septic tank, all as indicated in the
attached letter dated 4/24/14 and workplan drawing dated received in MHD City on 5/14/14.
Sedimentation and 'Erosion Control
4 2) In order to protect water quality, runoff from construction shall not visibly increase the amount of

,-,- ,'Appropriate sedimentation and erosion control devices, measures or structures shall be implemented to
ensure that 'eroded materials do not enter adjacent wetlands, watercourses and property (e.g. silt fence,
'diversion swales orlberms, etc.).

,-,,-Mis minor modification shall be attached to the original Permit No. 11-13, which was issued on
