Dear Mr. Odom:
In accordance with your permit major modification request received February 1, 2022, and
subsequent additional information received May 11, 2022, we are forwarding herewith a modified
Attachment B certified July 26, 2022, to the Town of Taylorsville for the subject residuals management
program.
Modifications to the subject Attachment B are as follows: 10.9 acres of Class B land application
area; and a setback waiver for the property and residence under Yolanda T. Prince.
The modified Attachment B shall be effective from the date of certification until November 30,
2027 (i.e., expiration date of the current permit), shall replace Attachment B of Permit No. WQ0006906
issued July 13, 2021, and shall be subject to the conditions and limitations as specified therein, as well as
those specified in the above-referenced permit. Please replace Attachment B of Permit No. WQ0006906
that was issued July 13, 2021 with the modified Attachment B. Please note that the Permittee shall
submit a renewal application no later than June 3, 2027.
Please note that only the maps for new land application sites are attached to this letter. It is the
complete permit is created by inserting these documents into the
previously issued permit.
If any parts, requirements, or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
