#2. Construction Sequence:
1. Working off-line, excavate new channel and install in -stream structures. Stockpile soil between new channel
and existing channel. Stabilize banks with transplants and/or mat.
2. Remove soil at upstream end of new channel to allow stream to flow into new channel.
3. Plug existing stream with excavated soil from new channel. Stabilize plug and fill in existing stream channel
with transplants and/or mat, seed and straw mulch.
4. Plant all disturbed area with seed, live stakes, and bare roots.

On May 18, 2022, the Division of Water Resources (Division) received your application requesting a 401 Water Quality
Certification from the Division for the subject project. On May 20, 2022, the Division received the required fee. The
Division has determined that your application is incomplete and cannot be processed. The application is on -hold until all
of the following information is received:
1. Please show the location of all proposed in -stream structures, riffles, and pools on the site plan for the new
proposed channel. For each proposed structure type (cross vane, J-hook vane, toe wood), provide a proposed
longitudinal profile and a proposed typical cross-section with dimensions. [15A NCAC 02H .0506 (b)]

2. Per condition II. 8 of Water Quality Certification 4255 all work in or adjacent to streams shall be constructed so
that the flowing stream does not come in contact with the disturbed area. Please provide a construction
sequence that details the method of diversion and/or dewatering to be utilized at the site and the sequence of
construction events to be followed to ensure compliance with this condition. [15A NCAC 02H .0506(b)(1) and
15A NCAC 02H .0502(a) and (c)]
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please provide your response by July 15, 2022. If all of the requested
information is not received, the Division will be unable to approve the application and it will be denied as incomplete.
The denial of this project will necessitate reapplication to the Division for approval, including a complete application
package and the appropriate fee.
