This letter is a reminder that the above referenced permit is set to expire on 12/31/2022.Per 15A NCAC 02T.0109,
a renewal application should have been received 180 days prior to expiration,which in this case was no later than
7/4/2022. Since this date has passed,please respond immediately—failure to submit the renewal application
prior to the expiration date listed above may result in enforcement action being taken. The application for the
renewal can be downloaded using the following steps:
• Go to:https://deq.nc.gov/about/divisions/water-resources/water-resources-permits/wastewater-
branch/collection-systems/system-wide-collection-system-permitting
• Click on Wastewater Collection System Application—New/Renewal(Form CSA 04/16) •
The original application,signed by an authorized signing official,with requisite supporting documentation as well
as one digital copy of the complete application packet(on a CD or flash drive)must be submitted to the following
