The purpose of this inspection was to ensure compliance with the conditions and limitations specified in Non -discharge Permit No. WQ0007026. During the inspection, the cooperation and assistance of Mr. Randall Jarrell, ORC, and Mr. Skot Hancox, Director of Plant Operations, was greatly appreciated. This compliance evaluation inspection consisted of the following: - Review of the permit, Review of ORC logbooks, calibration documentation, non -discharge monitoring reports (NDMRs), non -discharge application reports (NDARs) and On -site inspection of the treatment components and spray fields. The current permit was issued March 2, 2017 and expired February 28, 2022. The renewal application was received by the Division on April 18, 2022. Randall Jarrell is designated as Operator in Responsible Charge (ORC) and Chad Leinbach is designated as backup ORC. Components of the wastewater treatment and irrigation system were observed. The septic tanks are typically pumped annually and were pumped in the spring. The storage pond had plenty of freeboard available. The irrigation field appeared to be in good condition with good vegetative cover and no evidence of ponding or runoff The grass on the spray fields and surrounding the storage pond is routinely mowed. Each of the 5 sections of the spray field was activated and appeared to overall have good spray patterns and coverage. Monitoring wells MW-1, MW-4, and MW-5 were observed to be secured and well casings appeared to be in good condition. NDMR records were available and complete; lab results were available and compared to NDMR forms for June 2021 and March 2022. NDAR and GW-59 records were available and complete. The flow meter was last calibrated August 24, 2021, and the most recent annual representative soils analysis was conducted August 16, 2021. Please continue to ensure the flow meter is calibrated at least once per year (permit condition IV. 3.) and the annual representative soils analysis is conducted for this year (permit condition IV. 12.). The following permit condition violation was observed during the inspection. III. OPERATION AND MAINTENANCE REQUIREMENTS Condition 1 states "The facilities shall be properly maintained and operated at all times. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility." At the time of the inspection, approximately one quarter of the sand filter surface was ponded with wastewater. During the previous inspection conducted by RRO staff in 2020, a violation was generated due to ponding across the whole surface of the sand filter, and no response was received from the facility. Please submit a plan of action for repairing or replacing the sand filter. This facility was found to be non -compliant with permit conditions for failure to properly operate and maintain the treatment system as indicated by the observed ponding on the sand filter, and as a result, this inspection is being issued as a Notice of Violation. You are required to provide a written response with planned actions to return the facility to compliance to this office no later than 30 days after receiving this report.
