SUBJECT: Widening for turn lane at intersection of SR 1238 (Koonce Fork Rd) and US 258/24,
Onslow County Project Number: 3.206711
Dear Mr. Hennessey:
Please find enclosed two copies of a PCN application from the North Carolina Department of Transportation
requesting a Nationwide 18 permit for a safety improvement project on SR 1238 (Koonce Fork Rd.) in
Onslow County.
The scope of the project will be to construct a turn lane on SR 1238, which will require widening the existing
roadway approximately 11 ft for a distance of 500 ft. This work will be completed within the existing Right
of Way (R/W), but will require piping the lateral ditch with 18" RCP and the construction of 4-drop inlets.
In addition, the existing 30" CMP cross line under SR 1238 at the intersection of US 258/24 will need to be
extended 40 ft to north to modify the turning radius. This 40-ft pipe extension to the north of the intersection
will impact an intermittent tributary to the New River. The extension of the existing turn lane shown on the
attached plan sheet on the northbound lane of US 258/24 is required for the relocation of the driveway for
Woodson High School and will also be constructed as part of this project. This section of the project will
have no impacts on jurisdictional resources.
Total impacts for this project include 40 linear feet of an intermittent tributary to the New River, which is
designated as CSw NSW waters by the Division of Water Quality (DWQ). This impact is associated with a
30" pipe extension on the north side of the intersection of SR 1238 and US 258/24. There are no wetland
impacts associated with this project.
This office has conducted a GIS and field evaluation for T&E species and habitat within the project area and
is recommending a finding of "no effect". This project is within existing NCDOT R/W however a GIS and
field survey was conducted for historical and archaeological sites adjacent to the R/W and a determination of
"no effect" is recommended for historical resources as well,
Through coordination with the U.S. Army Corps of Engineers (USAGE) it has been determined that this
project can be authorized under Nationwide 18 and will not require preconstruction notification to the
USAGE. Therefore, in accordance with the corresponding Water Quality Certification 3631 we aze hereby
providing DWQ with two copies of the PCN application for review.

standard road construction equipment.

VI. Proposed Impacts to Waters of the United StateslWaters of the State
It is the applicant's (or agent's) responsibility to determine, delineate and map all impacts to
wetlands, open water, and stream channels associated with the project. Each impact must be
listed separately in the tables below (e.g., culvert installation should be listed separately from
riprap dissipater pads). Be sure to indicate if an impact is temporary. All proposed impacts,
permanent and temporary, must be listed, and must be labeled and clearly identifiable on an
accompanying site plan. All wetlands and waters, and ail streams (intermittent and perennial)
should be shown on a delineation map, whether or not impacts are proposed to these systems.
Wetland and stream evaluation and delineation forms should be included as appropriate.
Photographs may be included at the applicant's discretion. If this proposed impact is strictly for
wetland or stream mitigation, list and describe the impact in Section VIII below. If additional
space is needed for listing or description, please attach a separate sheet.

VII. Impact Justification (Avoidance and Minimization)
Specifically describe measures taken to avoid the proposed impacts. It may be useful to provide
information related to site constraints such as topography, building ordinances, accessibility, and
financial viability of the project. The applicant may attach drawings of alternative, lower-impact
site layouts, and explain why these design options were not feasible. Also discuss how impacts
were minimized once the desired site plan was developed. If applicable, discuss construction
techniques to be followed during construction to reduce impacts.
Proiect was desisrrled to use existing R/W to avoid impacts to adiacent property. Pine
lenalits were reduced as much as possible without jeopardizing safety & stability .
Strict enforcement of sedimentation & erosion control BMP's will be enforced.
VIII. Mitigation
DWQ - In accordance with 15A NCAC 2H .0500, mitigation may be required by the NC
Division of Water Quality for projects involving greater than or equal to one acre of impacts to
freshwater wetlands or greater than or equal to 150 lineaz feet of total impacts to perennial
streams.
