Dear Mr. Nichols,
In accordance with North Carolina General Statute § 90A-37, the Water Pollution Control System
Operators Certification Commission hereby classifies the subject facility as a Grade 4 (WW4)
Biological Water Pollution Control System effective today.
As required by North Carolina Administrative Code 15A 8G .0201 and your permit:
• you must designate one Operator in Responsible Charge (ORC) who holds an active
Grade WW4 Biological Wastewater Operator certification;
• you must designate at least one Backup ORC who holds an active Grade WW3 or higher
Biological Wastewater Operator certification; and
• if certified, you may designate yourself and/or your employees, or you may engage the
services of contract operators or a contract operation firm.

• contract operators
• training opportunities
• exam dates, locations, and eligibility requirements
• operator designation form
You need to:
1. within 60 days, complete and return an Operator Designation form signed by the ORC,
the Backup ORC, and you or your designee. You can mail a signed copy or email a
scanned signed copy. I've included the form with this letter or you can download a new
one from our website.
2. mail or fax a copy of that letter to the DEQ Regional Office in Wilmington. That contact
info is on the form I included.

If construction of your system is incomplete or has not yet begun and no waste has been
introduced into the system, you may notify this office, in writing, of an anticipated startup date
and your plans for obtaining and designating the required certified operators. Please note that
failure to designate a properly certified ORC and Backup ORC at least 60 days prior to the
introduction of waste into the system is a violation of Rule 15A NCAC 8G .0201(3) and your
permit.
