- Outlet of culvert needs to be stabilized. Impervious dike needs to be rebuilt/repaired and sediment directly above it
should be removed (This is within the footprint of the impacts). Recommend cover on slopes of the downstream
headwall to prevent additional loss into work area.
- Outlet of culvert - Right Bank Descending - Silt fence outlet at top of bank needs maintenance.
- Stock piles need to be completely covered.
- Before the water is turned into the box culverts, any sediment from the site that is in the culvert will need to be
removed.
NPDES records were up to date.
