Dear Mary S. Ford:
This letter is to inform you that the subject facility is operating a wastewater treatment system without a valid permit. The
Division of Water Resources records indicates Mary S. Ford currently owns the above subject facility. As the property
owner, you are also the owner of the existing single-family wastewater treatment system, which treats the domestic
wastewater from the residence and releases the effluent to waters of the state. According to Division records, a
NCG550000 Permit was originally issued on August 16, 2013. The Permit was issued to Melissa T. Nolan. The permit
fees have not been paid and the required Ownership Change Form has not been received.
This places Mary S. Ford in violation of North Carolina General Statute § 143-215.1(a)(2), which states that no person
may operate a treatment works or disposal system unless that person has received a permit from the Commission. Failure
to maintain a permit for the subject facility may result in the assessment of civil penalties of up to $25,000 per violation.
The Permit is no longer active because of non-payment of fees for the past seven years (2016, 2017, 2018, 2019, 2020,
2021, 2022). For your convenience, I have attached the past due invoices.
