Per Permit Condition IV.7, an annual report of all residuals production activities for a
calendar year must be submitted by March 1st of the following year. To date, an annual
report has not been submitted for this facility. This is a violation of the subject permit.
Please submit an annual report immediately or contact the Winston Salem Regional
Office for assistance.
As a reminder, the subject permit also expired on December 31, 2021. Please submit a
permit renewal application as soon as possible. The renewal application is enclosed and
can be requested electronically. If you have any questions about permit renewal or to
inquire about other options, please contact Caitlin Caudle in the Winston Salem Regional
