North Carolina General Statute 143-215.6E (1) provides that: Any person who knowingly makes any false statement,
representation, or certification in any application, record, report, plan, dr other document filed or.required to be maintained
under this Article or a rule implementing this Article ... shall be guilty of a Class 2 misdemeanor which may include a fine not
to exceed ten thousand dollars ($10,000).1 hereby request exclusion from NPDES stormwater permitting.

Regarding NPDES Stormwater rescission request for NCG 140210:
Chandler Concrete had leased the property from the rock quarry and ceased concrete production
operations in June 2019. To my knowledge, the rock quarry located at the same address, 1666 NC-49
Haw River, is still in operation. This rescission request only applies to Chandler Concrete's former
production operation in Haw River, not the rock quarry.
