SUBJECT: Request Stormwater COC Modification: change Outfall #001 location

Enclosed are the required supporting documents for the above referenced facility; requesting a
modification to the site's Stormwater Certificate of Compliance NCG030687 information, as covered
under North Carolina NPDES General Permit NCG030000. The Altec Industries, Inc. 200 Altec Way
facility has acquired additional property, which will move Outfall 001 to a new location.
The new location of Outfall 001 (36°27'28.54"N, 80°33'7.03"W) is a representation of discharges from the
previous Outfall 001 (36°27'36.99"N, 80°33'1.27"W) in addition to the newly acquired property. No change
is needed for Outfall 002 (36027'29.85"N, 80°32'56.51 "W).
Facility information: Altec Industries, Inc. owns and operates the facility that serves as a manufacturing
assembly and service operation in Mount Airy, North Carolina (Altec Mt. Airy). The facility is located at
200 Altec Way, Mount Airy, North Carolina, in Surry County. The facility services aerial lift trucks, and
assembles chassis and trailers for use by utility companies and the tree care industry.
The facility is located approximately 1,300 feet southwest of Reeves Mill Road and is approximately 4.5
miles southeast of downtown Mt. Airy, North Carolina. Altec Mt. Airy's property is now comprised of 108
acres and includes three single -story metal buildings originally built in 1998. The Mt. Airy-Surry County
Airport borders the western property boundary and Pike Electric, LLC property borders the northeastern
property boundary.
