Description of Complaint: HDR reported to Neil Uldrick on December 3, 2021, about an outfall
with green benthic growth, gray cloudy water, and a smell of sewage. Neil sent this information to
stormwater staff on December 3, 2021 for further investigation.
Action Items to be Resolved: Stormwater arrived to investigate on December 3, 2021 at 2:00 PM.
The outfall had a gray cloudy appearance, but no smell of sewage. YSI parameters were measured,
and they were in a safe range. Staff then opened a storm drain manhole, and a sewer manhole that
were 20 ft away from the outfall to see any evidence of a sewer leak. The storm drain fed the outfall,
had active discharge, and is fed by storm drains on the corner of Nelson St and Devonshire St, and by
storm drains on Sprague St. There is also a sewer line that runs across the creek below the outfall, that
comes from Brookline St. Stormwater staff dyed this sewer line, and the sewer line on Sprague St and
Nelson St. The dye was not observed in the storm drain manhole, or in the outfall/creek. Staff then
left the location. Stormwater staff returned to finish its investigation on December 6, 2021. Staff
arrived to the location at 9:00 AM. Staff dyed a sewer manhole further up the street on Sprague St,
and the sewer manhole cover 20 ft from the outfall. YSI parameters were measured to be the same as
December 3, 2021 while staff waited for dye to show up at the outfall. The dye was not observed and
there was no smell of sewage. Staff then left the site.
Result: These results will be returned to Neil Uldrick for HDR to look over.

Description of Complaint: Stormwater staff was alerted to an outfall pipe with dry weather
discharge on December 3, 2021, by HDR. Staff initially investigated this discharge on December 3,
2021 and found cloudy water trickling out of the outfall pipe. YSI measurements were gathered this
day. The parameters were as follows:
• Temperature: 14.7 °C
• Conductivity: 137.2 µS/cm
• Total Dissolved Solids: 111.15 mg/L
• Dissolved Oxygen: 90.0 % ; 9.14 mg/L
• pH: 6.83 S.U.
• Ammonia: 0.00 mg/L
Stormwater staff returned to the site on December 6, 2021, to confirm there was not a sewer leak on
account of the cloudy water, and smell of sewer described on the HDR investigation. YSI parameters
were gathered again this day and are as follows:
• Temperature: 15.0 °C
• Conductivity: 124.7 µS/cm
• Total Dissolved Solids: 100.10 mg/L
• Dissolved Oxygen: 56.5 % ; 5.60 mg/L
• pH: 6.62 S.U.
• Ammonia: 0.00 mg/L
Stormwater staff dyed the sewer lines that crossed this stormwater pipe on Devonshire Street, and
again at multiple locations up the hill of the outfall on Sprague Street. No dye was ever observed at
the outfall. Ammonia and fecal coliform samples were grabbed and taken for lab analysis at Pace

Analytical on December 6, 2021. Ammonia was non -detectable (ND) from the outfall, and the fecal
coliforms were measured at 5200 cfu/100 mL. Winston-Salem Utilities was alerted that there may be
a sewer leak with low flow, due to no dye being found, but no leak was ever detected. The outfall
would never appear cloudy again during any site investigations. All site investigations were
conducted during dry weather.
During the investigation with Winston-Salem Utilities, Stormwater staff found a house at 530 W
Sprague Street with a dog lot and a storm drain in the back yard. There was dog feces laying on the
ground of the backyard, in and around the dog lot, that would pollute the storm sewer system in the
event of a rain event. This could be a possible source for the elevated fecal coliforms at the outfall.
This investigation was sent to the City of Winston-Salem Community Development Department due
to the poor conditions found inside the house at 530 W Sprague Street. The Community Development
Department issued the property owner a notice of violation on January 20, 2022, requesting them to
clean the dog feces from the backyard. The owner complied with the violation, cleaning up the
backyard, and the case was closed on March 3, 2022.
After this possible pollutant source was cleaned, Stormwater staff waited for a rain event to flush out
the remaining possible pollutants from the pet waste in the backyard of 530 W Sprague Street.
Another fecal coliform sample was taken to Pace Analytical on March 28, 2022 and measured to be
5400 cfu/100 mL. Since the fecal coliform level was not decreasing overtime, stormwater staff
decided to do another Pace Analytical lab analyzed grab of fecal coliforms and ammonia on April 11,
2022. Ammonia was non -detectable (ND) and fecal coliforms had decreased but were still measured
to be 2600 cfu/100 mL. On May 6, 2022, stormwater did another sample grab of ammonia, nitrate,
and fecal coliforms. Pace Analytical lab analysis found that ammonia was measured to be 0.12 mg/L,
nitrate was 4.2 mg/L, and fecal coliforms were 6400 cfu/100 L.
Action Items to be Resolved: Stormwater staff returned to the site once again on May 12, 2022. YSI
parameters were measured along with grab samples of ammonia, nitrate, and fecal coliforms. YSI
parameters are as follows:
• Temperature: 15.4 °C
• Conductivity: 718.9 µS/cm
• Total Dissolved Solids: 113.10 mg/L
• Dissolved Oxygen: 58.7 % ; 5.65 mg/L
• pH: 6.13 S.U.
• Ammonia: 0.00 mg/L
Grab samples were taken to Pace Analytical. Ammonia was non -detectable (ND), nitrate was found
to be 4.1 mg/L, and fecal coliforms were 8200 cfu/100 L. The water at the outfall was stagnant,
moving at a trickle. The water was clear, had no color, had no sheen, and had no smell. Stormwater
staff then tracked the outfall pipe upstream to storm drains on Sprague Street and found a small
trickle of water running through them. No obvious signs of an illicit discharge were found in these
drains, upstream of the outfall, to cause the elevated fecal coliform measurements found at the outfall.
The low dissolved oxygen measurements at the outfall are likely due to the stagnant water and high

fecal coliform count. Due to the consistently low ammonia measurements, which would not be
expected of a sewer leak, and yet consistently high fecal coliform counts, the cause of these
parameters is likely not a sewer leak but instead the result of sheet flow from the surrounding
neighborhood and dog lots. There are two dog lots directly adjacent to the outfall, as well as dense
housing directly uphill of the outfall along with dense housing.
Result: Illicit discharge identified and found to be pet/animal waste
