The nitrogen concentrations in table in Section III.4.a. of the application are less than half what
was in the June 1, 2021 renewal application (for example the First -Year Surface application average
was 27.42 pounds of PAN per dry ton (lb PAN/DT) of residuals in the renewal, but only 11.68 lb
PAN/DT in this application). The residual source in this permit has not changed with this
application, and the treatment plant is a municipal treatment plant that shouldn't have such a
variable nutrient concentration. Please provide an explanation for the discrepancy.
D. Operation and Maintenance Plan (O&M Plan):

F. Land Application Site Certification (FORM: LASC 06-16) Form:
1. The Standard Soil Fertility Analysis required in accordance with 15A NCAC 02T .1104(c)(2)(B)
was not provided for field NC-PI-06-01. There was an analysis provided for the metal pollutants
listed in 02T .1105(b) as required by this rule, but not an analysis for the parameters listed in the
second part of this rule. Please provide the Standard Soil Fertility Analysis for this field.
2. The Washington Regional Office observed a cemetery in the northern area of field NC-PI-06-01 in
their site visit. While this is owned by the landowner of the field, it should still be shown in the site
map. Please provide an updated field application site map which denotes this private cemetery.
