The City makes no guarantee of adequate public right-of-way to support the proposed installation as shown on the submitted plan. This is the responsibility of the applicant. Note that this is not an authorization to construct. Any installation or disturbance within the public street or within public right-of-way will require a preliminary field meeting with a representative of the Engineering Department before authorization to construct will be given. All work that requires an adjustment to normal traffic flow shall be performed in a manner that complies with FHWA traffic control practices as noted in the MUTCD manual. Pedestrian and vehicular traffic must be maintained at all times. Please note day and time restrictions (if any) noted on the attached ROW use application. It is the applying utility's responsibility to locate all other utilities prior to installation. All utilities shall be a minimum of 30" below the nearest roadway surface —or- depth required by the governing utility code. The new installation shall be a minimum of 48 inches measured perpendicularly from the center of any City maintained utility and 24" clear separation when crossing any City maintained utility. The utility owner shall agree to remedy any possible conflicts regarding the permitted utility, which the City may encounter while performing maintenance of a City owned utility, with no cost borne by the City. The utility shall agree to restore all infrastructure in accordance with applicable City of Burlington specifications. USACE cannot properly address this issue without sampling off of the TAMP facility. Wells installed during the assessment will be completed as temporary or flush mount monitoring wells that will be completed at the existing grade so that no vertical obstruction will be left. The US Government will abandon any permanent monitoring wells at the end of the assessment and any requisite remediation.
