1.0 INTRODUCTION
1.1 Site Location, Description, and History
The Southern States Cooperative, Inc. (Southern States) fertilizer plant (Site) is at 2582 Salisbury
Highway, approximately 6 miles east of Statesville, North Carolina (Figure 1). The Site is
developed with several buildings that include a fertilizer -manufacturing building, an office
building, a warehouse, several support buildings, and multiple aboveground storage tanks
(ASTs; Figure 2). The Site is used for the manufacture, storage, and distribution of fertilizer. The
Site is bordered to the north by Salisbury Highway; to the east by McNess Road, across which
are Ferti Technologies and Cardinal Logistics Management; to the south by U.S. Highway 70
Bypass (70 Bypass), across which are single-family residences; and to the west by agricultural
land. The 70 Bypass crosses east to west through the southern portion of the Site and separates
approximately 13 acres from the main property that consists of approximately 61 acres to the
north.
Groundwater at the Site exhibits ammonia and nitrate from releases of liquid ammonium
nitrate that apparently flowed into four surface -water ponds formerly on the Site. Two ponds
were formerly located approximately 1,000 feet southwest of the main plant, and two ponds
were formerly located east and southeast of the main plant. Those ponds are referenced in the
Corrective Action Plan (CAP) dated January 28, 1994, submitted by Canonie Environmental
Services, Inc. (Canonie) to the North Carolina Department of Environment, Health and Natural
Resources (NCDEHNR), Division of Environmental Management (now called the Department of
Environmental Quality, or NCDEQ). The NCDEHNR approved the CAP in 1994. The CAP showed
the extent of affected groundwater was defined with reasonable certainty. The objectives of
the CAP were to remediate groundwater and to prevent off -Site migration of the constituent
plume. Canonie developed a groundwater -sampling plan that established a semiannual
groundwater -sampling schedule to monitor the effectiveness of the CAP.
In an email dated April 30, 2014, to Duncklee & Dunham (now SynTerra) from Andrew Pitner,
NCDEQ agreed with our recommendation in the 2013 Annual Groundwater Monitoring and
Treatment System Effectiveness Report to reduce the frequency of groundwater monitoring
from semiannual to annual.
On behalf of Southern States, Duncklee & Dunham researched nitrate -treatment technologies,
evaluated remedial alternatives for the subject Site, and reported findings to NCDEQ in the
2015 Annual Groundwater Monitoring and Treatment System Effectiveness Report, dated
November 9, 2015. We recommended in the report that the continued operation of the existing
treatment system appeared to be the most cost-effective approach to remove constituent mass
from groundwater and control off -Site migration of the plume. We met with NCDEQ personnel
at the subject Site on December 9, 2015, to discuss the findings and remedial alternatives
presented in our 2015 report.

During this meeting, NCDEQ requested and Southern States agreed to:
• Begin semiannual sampling of the extraction wells
• Collect vertical -gradient data from the monitoring wells that surround RW-2
• Extend the operational period of the treatment system to throughout the year.
1.2 Summary of Remedial Actions
Groundwater -recovery wells RW-2, RW-5r and RW-6 were constructed as part of the CAP.
These wells pump groundwater to a treatment system, in which the pH is adjusted by adding a
caustic solution to the water prior to discharge from Outfall 001 into Fourth Creek (Figure 3).
The groundwater -treatment system is permitted under the National Pollution Discharge
Elimination System (NPDES) program. SynTerra conducts bi-monthly Site visits to maintain and
monitor the treatment system. We collect effluent water samples bi-monthly and report the
data to NCDEQ on monthly Discharge Monitoring Reports (DMR).
TREATMENT -SYSTEM MAINTENANCE AND MONITORING
The groundwater -treatment system was operational during the reporting period from January
1, 2021, to December 31, 2021. The treatment system did not experience shutdowns or
malfunctions during this time period.
The table below summarizes the volumes of water removed by each extraction well during this
reporting period.

SynTerra did not collect water samples from the three recovery wells during this reporting
period. Sampling of the three recovery wells will resume on a semi-annual basis during the
2022 reporting period. This sampling will include the collection of samples from the spigots
near the wellheads, which will be submitted for laboratory analysis of nitrate according to the
United States Environmental Protection Agency (EPA) Method 300 and ammonia -nitrogen
according to SM 4500. A summary of these test results and copies of analytical laboratory
reports will be included in the next monitoring report.

In accordance with the NPDES permit, the operator collects treatment system effluent samples
bi-monthly for analysis of total suspended solids (TSS) according to Method SM 2540D and
ammonia -nitrogen according to SM 4500. The operator also collects monthly effluent samples
for analysis of copper according to EPA Method 200.7, total phosphorus according to SM4500,
total Kjeldahl nitrogen according to EPA Method 351.2, and total nitrate and chloride according
to EPA Method 300. In accordance with the current permit issued on August 1, 2019, the
operator also collects monthly effluent samples to be tested for total hardness according to EPA
Method 200.7. We submit the effluent samples to Waypoint for laboratory analysis. The
operator also collects effluent samples for analysis of Ceriodaphnia chronic toxicity (chronic
toxicity) according to the December 2010 North Carolina Phase 11 Chronic Whole Effluent
Toxicity test procedure.
SynTerra collected monthly samples to be tested for chronic toxicity. We submitted these
samples to Waypoint Analytical and submitted quarterly DMRs that include chronic toxicity
results to DEQ, as required by the NPDES permit.
3.0 MONITORING WELL -SAMPLING ACTIVITIES AND RESULTS
SynTerra conducted annual groundwater -monitoring activities on November 2, 3, and 5, 2021.
Prior to purging and sampling each well, we measured depths to the water table with an
electric water -level probe and recorded those data on field -sampling data sheets. Table 2
summarizes those depths and respective water -table elevations. Figure 4 illustrates the
potentiometric surface for the November 2021 sampling event. The direction of groundwater
flow during the event was to the southwest and the average hydraulic gradient was 0.033
feet/feet, which is consistent with historical trends. The vertical -gradient from the monitoring
wells that surround RW-2 could not be calculated because K65W7F and K65W7G were both dry
at the time of sampling.
3.1 Monitoring Well -Sampling Methodology
SynTerra collected groundwater samples from 10 of the 14 monitoring wells during the
November 2021 sampling event. We did not sample K65W6D because this well is damaged and
has historically been dry during sampling events since February 1993. K65W7C is damaged and
has remained dry since April 2011. Monitoring wells K65W7F and K65W7G were dry during this
event.
SynTerra purged the wells and collected samples using a peristaltic pump fitted with new
tubing to sample those wells in which the depth to water was less than 26 feet below top of
casing (btoc). We collected the groundwater samples using low-flow/minimum draw -down
sampling techniques. During the sampling process, we recorded field measurements for
dissolved oxygen (DO), oxidation-reduction potential (ORP), pH, temperature, turbidity, and
electrical conductivity, as summarized in Table 3.

SynTerra discharged the groundwater samples directly into labeled laboratory -provided
containers, recorded the samples on a chain -of -custody form, placed the containers in coolers
with ice, and delivered the coolers to Pace. Pace tested the samples for nitrate according to
USEPA Method 300.0 and ammonia nitrogen according to USEPA Method 350.1.
3.2 Results from Sampling Event
The laboratory reports are in Appendix A. Table 3 summarizes the test results and graphs of the
nitrate and ammonia trends over time are in Appendix B. Nitrate isoconcentration maps are
presented as Figures 5 and 6. Nitrate and ammonia concentrations are displayed on cross
sections A -A' (Figure 7) and B-B' (Figure 8).
3.2.1 Nitrate Results and Trends
We observed the following with respect to the nitrate results.
• Samples from saprolite wells K65W6A, -6B, -6N, and -7J, and samples from
bedrock wells K65W6E, -7K and -7L exhibited nitrate at concentrations greater
than the North Carolina groundwater quality standard (2L standard) of 10
milligrams per liter (mg/L).
• The nitrate concentration in the sample from saprolite well K65W6A, located in
the core of the plume, decreased from 140 mg/L to 99.6 mg/L in November
2021. The nitrate concentration in the sample from the bedrock well K65W7K
decreased from 94 mg/L to 31.3 mg/L, which is the lowest detection in a sample
since 2007. The nitrate concentration in the sample from saprolite well K65W7J
decreased from 110 mg/L during the previous sampling event to 65.1 mg/L. The
nitrate concentration in the sample from bedrock well K65W7L decreased from
120 mg/L to 99.8 mg/L.
• Samples from bedrock monitoring well K65W6E, located near the former
location of Pond B, exhibited nitrate at a concentration of 384 mg/L, the highest
concentration detected in samples from this well. Samples from this well have
exhibited an increasing trend in nitrate concentrations since May 1997. The
nitrate concentration in the sample from saprolite well K65W6N increased from
19 mg/L during the previous sampling event to 130 mg/L in November 2021,
which is the highest concentration detected in samples from this well.
• The sample from bedrock well K65W6B, located in the northern zone of the
plume, increased from 41 mg/L to 45.9 mg/L and remain consistent with
historical concentrations.
• Samples from saprolite wells K65W6M and K65W67D and bedrock well K65W7H,
at the southern extent of the plume, did not exhibit nitrate at concentrations
greater than the 2L standard.

• In general, concentrations of nitrate in samples from monitoring wells have
exhibited stable to decreasing trends over time, with the exception of K65W6E
and K65W7G. However, K65W7G has been dry since 2016.
3.2.2 Ammonia Results and Trends
We observed the following with respect to the ammonia results.
• The sample collected from saprolite well K65W7J exhibited ammonia at a
concentration greater than the current Interim Maximum Allowable
Concentration (IMAC) of 1.5 mg/L. Ammonia concentrations in samples from
K65W7J have been greater than the IMAC since October 2010. The ammonia
concentration in the sample decreased from 5.1 mg/L during the previous
sampling event to 4.34 mg/L in November 2021.
• Samples from the saprolite well K65W6A exhibited ammonia at 0.173 mg/L, a
decrease from the previous concentration of 0.18 mg/L in October 2020.
• The ammonia concentration from saprolite well K65W6N decreased from 2.2
mg/L to 0.354 mg/L. The ammonia concentration from bedrock well K65W6N,
located in the center of the plume, decreased from 1.9 mg/L to 1.44 mg/L. The
ammonia concentration from saprolite well K65W7L decreased from 6.9 mg/L to
1.34 mg/L. All concentrations detected from these wells in November 2021
decreased to less than IMACs.
• Samples from bedrock monitoring well K65W6E, located near the former
location of Pond B, have exhibited a decreasing trend in ammonia
concentrations since November 2011.
• Samples from saprolite wells K65W6B, -6M, -7H, and -7D and bedrock well
K65W7H did not exhibit ammonia at detectable concentrations.
• In general, concentrations of ammonia nitrogen in samples from monitoring
wells have exhibited stable to decreasing trends over time, with the exception of
K65W7G and K65W7J.

4.0 RECOMMENDATIONS
Based on the information presented above, SynTerra recommends the following:
• Continue to operate the groundwater -treatment system in accordance with the CAP.
• Test groundwater samples from the three recovery wells for nitrate and ammonia on a
semi-annual basis and conduct the next event in June and December of 2022.
• Continue the groundwater -monitoring program on an annual basis and conduct the next
sampling event in October 2022.

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form
or properly qualified within the sample results. By my digital signature below, I affirm to the best of my
knowledge, all problems/anomalies observed by the laboratory as having the potential to affect the
quality of the data have been identified by the laboratory, and no information or data have been

Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On -Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.

Sample Delivery Group.
Not detected at the Reporting Limit (or MDL where applicable).
The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample
The non -spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state "ND" (Not Detected) or "BDL"
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)
Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non -conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss) This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.

Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On -Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.

The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample
The non -spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state "ND" (Not Detected) or "BDL"
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)
Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non -conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss) This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form

GLOSSARY OF TERMS
Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On -Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.

Sample Delivery Group.
Not detected at the Reporting Limit (or MDL where applicable).
The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Original Sample
The non -spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control
sample. The Original Sample may not be included within the reported SDG.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state "ND" (Not Detected) or "BDL"
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)
Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non -conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss) This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
