Subject: Request for aNW 3Permit and DWQ 401 for the replacement of over Unnamed
WBS # 17BP.1.R.46
Tributary to Perquimans Riveron SR 1001(TurnpikeRoad)in PerquimansCounty.
The North Carolina Department of Transportation is requesting the issuanceof aNW 3permit
and corresponding DWQ certification for the replacement of Low Impact Bridge #25in Perquimans
County.The bridge is located on SR 1001approximately 7.0miles southeastof the intersection of NC
Highway 158and spans an Unnamed Tributary to Perquimans River.The receiving streamfed by the
unnamed tributary is classified by DWQ as class C;Swwaters. Bridge #25is currently asingle span pre-
stressed concrete channel deck on concrete caps & timber piles; OAL = 30’.This structure will be removed
using NCDOT’s guidelines for bridge removal. The proposed bridge will be placed on its existing
alignment. The new bridge structure will be asingle-span 18”cored slab typebridge;OAL = 45’ long by
30’ wide.The proposed bridge will have vertical sheet pile abutments at the end bents. Placement and
construction of the proposed bridge end bents, caps and associated roadway fill will result in<0.01 acres of
permanent wetland impact. Structural sheet pile and associated fill from the vertical sheet pile retaining
wall results in 95linear feet of permanent stream impacts.An off-site detour will beutilized during
construction.This bridge is following the Low Impact Bridge Processestablished by FHWA, NCDOT,
and the Regulatory Agencies.

Project Description
–
The new bridge structure will be a single-span 18” cored slab type bridge; OAL
= 45’ long by 30’ wide. The proposed bridge will have vertical sheet pile abutments at the end bents.
There are no jurisdictional wetlands present on site. Placement and construction of the proposed bridge
end bents, caps and associated roadway fill will result in <0.01 acres of permanent wetland impact.
Structural sheet pile and associated fill from the vertical sheet pile retaining wall results in 95 linear feet of
permanent stream impacts. An off-site detour will be utilized during construction.This bridge is following
the Low Impact Bridge Process established by FHWA, NCDOT, and the Regulatory Agencies.
