I wanted to forward the letter requesting the Hideaways WWTP (NC0057401) to be placed on an inactive status. Don't
worry about the ORC/Backup ORC designation form attachment, that should have been done several months ago. Just
an FYI, I have no objections to placing this facility on an inactive status, I was there a few weeks ago to verify the
facility's status.

This attached is one of two forms I need your help on asap. Please fill in the blanks or make
corrections as you see appropriate, then sign and email back to me being sure to "reply all".

This form is periodically updated to ensure the State has the right contact information for both
owner and operator. Greg, my back up operator and I have already signed.
A second and totally different form will come next week under separate cover also via fax. This
form/letter is necessary to request remission from fees/violations associated with late
reporting during my recent battle with Covid and need to office quarantine. I will coach you
through the it's completion at that time. To help me serve your needs more promptly....We will
be sure to include a statement that allows the State to copy Metwater on issues related to your
facility so that I may see what you see... when you see it ...not day or weeks later.
