Dear Sir/Madame:
The information accompanying this cover letter is submitted per North Carolina Division of
Energy, Mineral, and Land Resources, Industrial Storm Water General Permit NCG030000,
Certificate of Coverage number NCG030557, Part E. Included, as required, are the original
paper copy of the Discharge Monitoring Report (DMR), and a copy of the DMR Upload Form.
During the monitoring period, there was no measurable stormwater discharge at either of our
site outfalls during business hours or unsafe conditions existed during the storm event. The
enclosed documentation applies to the following time -period:
Year: 2022 1 Quarter: I First 1 /l/22 — 3/30/22 3rd DMR of permit term — No Flow
Furthermore, our storm water outfalls have had Qualitative Monitoring visual inspections, which
are recorded and filed per Part D requirements. The two outfalls at our site have both been
evaluated for non-stormwater discharges and sources of pollution as well as the list of
parameters found in Part D-1 (d), i thru x.
